Hello, this is Dermatologist Dr. Kwon Inhyuk. Hello, this is Dermatologist Dr. Park Jaebum. [Acne Scars and Devices] Today, we will discuss acne scars and introduce a new device that is used for treating acne scars. Since we perform many acne scar treatments,

though we've used all the devices available up to this point, we felt that this new device is certainly unlike the others. We used it for around two months and felt that it was a groundbreaking treatment method so I will talk about it together with Dr. Park. [Acne Scar Treatment and Development Process] Firstly,

[Acne Scar Treatment and Development Process] we'll share how treatments have been performed until now, [Acne Scar Treatment and Development Process] how they developed, [Acne Scar Treatment and Development Process] and how the new CureJet differs from existing devices. [Acne Scar Treatment and Development Process] Would you talk about these? I'll first briefly discuss the history of acne scar treatment.

There has been demand for acne scar treatment from long ago. The method they used in the past was to completely damage the skin, a.k.a. resurfacing. sometimes, they even physically damage off the skin. With chemical peels, they would apply strong acids like phenol to the face

to resurface the skin as a type of treatment. When it worked, the results were excellent. It's an all-or-nothing gamble. You would either become much better, or completely ruin your skin. So those methods were used in the past.

Following that, lasers began to develop to similarly resurface the skin. But that also had many issues. They were improved upon and the fraxel laser was made. When the fraxel laser is fired,

its beam is split up to minimize damage as explained. It still makes tiny wounds on the skin. So it may leave pigmented marks or cause scabs or discharge, making skin care difficult. So there were more developments

and the picofraxel laser came out, which is used often. Picofraxel doesn't damage the outer skin. When it's fired, it creates minuscule holes underneath the skin, and the skin fills out as it recovers, treating the scar. However, there was still one issue with this. And now there is a completely new device called CureJet

I'll summarize everything up until this point. In the beginning, there was method called dermabrasion. Resurfacing. Yes, resurfacing. Besides making skin sensitive,

it could be seen as a treatment with great results Next was fraxels like Erbium or CO2 that damaged and resurfaced the surface of the skin. After that was the pico fraxel method. So in conclusion, they were effective, but treatments evolved to make skin less sensitive.

Lastly, after the pico fraxel the next method to minimize skin damage is the CureJet. That's correct. This device newly came in and we've only used it for two months, but we've experienced a lot of things in those two months.

[What kind of device is CureJet?] Can you explain what kind of device CureJet is? CureJet is a device that injects drugs into skin without needles, but this also evolved many times. The first version that came out used an erbium laser which used force to inject medication, called Mirajet. This is also quite well-known.

This kind of device existed. The next generation used electric sparks, but it failed to launch as a device. Lastly, CureJet was launched as the third generation. This uses the solenoid method. Solenoids are a bit complex.

It's a coil with a magnetic field that's wrapped around. If there is an electric field, pressure forms. And if we use that pressure to expel the fluid, the fluid bursts out at a high speed, piercing into the skin and entering the scars.

So before when we treated scars, we used to use lasers and injectables together. When you inject fluid in scars, it's very easy when you treat large scars. But the better they get, the smaller they become. And there are icepick scars that are small and deep.

It's very hard to inject into these scars as they don't go in. So this was a roadblock once the scars healed up to a point. But as CureJet entered the market, the scars that couldn't be injected in the past can now accurately be targeted. Similarly for larger scars,

using CureJet in place of syringes has made the treatment more effective. In conclusion, CureJet makes needle-free injections possible. This technology that didn't exist was first developed in here Korea, right? Yes, that's right.

To my knowledge, Prof. Yeo Jae Ik was involved in the development of all 3 generations and is the president of the company. So, this kind of device was first developed in our country and is a device that injects fluids without needles. [What areas is CureJet mainly implemented in?] It can be used in many areas,

but how is it mostly used today? It's mostly used for scars as we mentioned up until now. But there are lesions that are similar to scars like stretch marks. It was very difficult to treat stretch marks until now. Stretch marks very often

when pregnant individuals' bellies grow rapidly and the skin stretches and tears, resulting in pitting. For treatment in the past, we would need to use strong lasers to damage the skin or individually insert a needle by hand.

We used those kinds of treatments, but they were difficult and not effective. But as CureJet emerged, since we can finely inject drugs into the pitted stretch marks, skin can regenerate and heal, making treatments much easier. [Mainly for scar and stretch mark treatments] So it's mainly used for scars and stretch marks.

Other than that, there's skin boosters. Before, skin boosters were also injected by hand. We can also inject skin boosters in the skin without needles or inject effective ingredients into the scalp as hair loss treatment, which has been shown to be effective. That's right, acne scars,

stretch marks, skin boosters, hair loss injections, and other treatment methods may be possible, but we mostly use it for acne scars and stretch marks. It's not like stretch marks couldn't be treated in the past,

but sometimes we have to treat a patient's entire belly. If we inject each stretch mark one by one, the procedure time is long and I've even taken up to an hour. But now that we have equipment like CureJet, it's more comfortable for the patient and much more comfortable for the operator.

So the procedure time decreases, and the pain is felt for a relatively shorter time. And also in shrunken scars or stretch marks where it didn't inject well this helps inject much more effectively than before. CureJet is a needle-free injector,

but if you hear this, [How painful is the CureJet procedure?] you may think that it doesn't hurt. [How painful is the CureJet procedure?] What is your opinion on the pain level? When their company advertises it, they advertise it as being minimally painful. You've tried it.

How was it honestly? It was quite painful, actually. [A bit painful] It was painful for me, too. [A bit painful] Our staff also tried it out [A bit painful] They also found it very painful at first. But it can't be helped

because it has to be forceful enough to pierce the skin. It uses strong pressure to enter the skin. But it's not just piercing the skin, it also tears the inner layer as it's injected. We can see better results if it tears the skin. So it can be quite painful.

On the other hand, it's less painful than using a thick needle for injection and a bit more painful than using a thin needle. As it feels like shooting a gun, it vibrates a bit. It really hurts when used on the nose bridge. Since it vibrates the bone.

[Painful in areas like nose bridge and forehead] Areas like the nose bridge and forehead. Actually, I can't endure pain so it's very painful for me. But there are people that take it really well. There are these people and [Pain adjustment possible with CureJet] we can control the pain level. More specifically,

CureJet can be performed painfully and less painfully. Would you share tips regarding this? If you look at the CureJet equipment, you can adjust the level. Level adjusts the strength and depth of injection.

When you use a stronger level, it will hurt more. When you reduce the level, you reduce the pain. You can adjust this. Each burst releases a certain amount of fluid. Of course when you shoot a large amount of fluid, it will inject more and hurt more.

If you reduce the amount, less will be injected and it will hurt less. There's no specific level. You shoot a few times on the patient and gauge how well they tolerate the pain and set it based on their sensitivity.

That's right. The injection strength, and the amount injected per shot can be used to adjust pain Also, when I try it, you can also adjust the distance.

There will be a difference when touching the skin versus shooting at a bit of a distance. It also depends on the angle. You can use it perpendicularly or at a slight angle. We can also adjust the subcision area based on these.

The practitioner will need this know-how. To summarize, It is a needle-free injector that can be minimally painful but can be quite painful with a stronger, more effective setting. We went over pain management of CureJet

and as this is a new device, it will have advantages and disadvantages, right? So you already mentioned the advantages in the beginning, but could you summarize them again? [CureJet Advanages] To explain the advantages again,

any type of scar, as long as it's pitted can be finely injected into. No matter how small, even as small as a pore, the fluid can be injected accurately. So even lesions that couldn't be treated before, can be accurately and effectively targeted.

Additional, as I mentioned briefly, it doesn't just inject medicine. I explained that the fluid spreads and tears the inside. In the past, subcision was a treatment method for scars. They used a thick needle called Nokor and stabbed the skin and sawed underneath the skin.

The scar fibers were cut by hand, but it was very painful and caused severe bruising and even caused hyperpigmentation if mishandled. But for this, I can precisely target the pitted area, inject the fluid, and tear the scar fibers, so there is a subcision effect

without overstepping the treatment area, accurately tearing the scar tissue. Due to these effects, [CureJet's Biggest Advantage: simultaneous injection & subcision] there is a huge advantage [CureJet's Biggest Advantage: simultaneous injection & subcision] of providing injectables and subcision. Medication injection

and subcision. And to add to the injection, depth. I think that injection depth is key. Actually, when we treated scars up until now, even if we wanted to use JuveLook Volume,

into our desired dermal layer, the papillary dermis, it was difficult to accurately target that layer. And there was no method to target that layer. For example, the needle itself is 0.2-0.3 mm in diameter, but to reach the desired layer, we need to reach 0.4 mm deep. But if it's too deep,

it spills into the subcutaneous fat layer. If it's too shallow, it doesn't inject If it's too shallow, it doesn't inject And spills out. So being able to inject an accurate dosage and depth with subcision is the biggest advantage. I think being able to target the papillary dermis

is a huge advantage with CureJet. But after using it, we know it doesn't just have advantages. Would you explain some of the disadvantages? [CureJet Disadvantages] CureJet's disadvantages will be similar to other scar treatments. As mentioned, it isn't pain-free. Not pain-free, so there is pain.

But this can be adjusted and overcome. It can also leave a few marks. Rather than a hole from a needle, there will be an area where the fluid enters the skin and there will be scabs like small red marks, but they don't last long.

We used CureJet on many patients and we ask each time. If we ask how long the scabs lasted, they would typically say 3-4 days. I think the average is also 3-4 days. And since they aren't large marks

they said it wasn't that inconvenient. With stronger settings, some experienced discharge from the holes for 1-2 days, but they all said it got better in a few days. Right, so I'd say that pain the the biggest disadvantage. It can be used to be minimally painful.

But truthfully, even today, When the acne scars are severe, especially with male patients whose scarred skin resembles an orange peel- With rigid scars -rigid scars and a long history of treatments,

I tend to start their treatment strong. When I get their feedback, not all of them said this, [It's the most painful treatment I've ever received!!] but some said it was the most painful treatment they've had. It may be because the treatment densely pierces hundreds of spots in the skin. When we used needles in the past,

we couldn't use it on 100-200 different spots. But now, with CureJet, we can target tiny icepick scars and enlarged pore-like scars. So we are more thorough, which may be more painful In the past, if there were hundreds of scars,

we would also get tired if we tried treating them all. So for scars that seemed unlikely to be treatable were skipped. But since we can target any scar with this, I become ambitious in trying to treat them all. I tell the patient to try to bear it as I target them all in order to make it more effective.

And as I mentioned before, I heard it was the most painful treatment, but compared to non-painful treatments they had before, the redness didn't last. So it heals faster. So for workers who get treated on Friday,

It's not obvious when they return to work on Monday. That's a big advantage. It's very painful, but it doesn't look like it. Though I'm not sure if that's a good or bad thing. But it does have good effects. As of now, it's the first of its kind developed in Korea

and there was Mirajet, the first and second generations, but it's still not a perfect device. So as the operator, the patients who received this treatment already know, as the procedure time gets longer, the jet stream must be formed again.

So sometimes the pressure drops and we have to re-form the jet, which actually this isn't a disadvantage for the patient, but it does become more complex for the operator. The jet doesn't just shoot into the skin if you turn it on. After stepping on the pedal for a little bit, the jet sort of bursts out.

Only then can we inject the fluid into the skin. But as you keep using the machine, the jet should be continuous, but sometimes it doesn't shoot out well. Right, it sounds different. You get the feeling if you've used it many times.

So then we must step back and re-form the proper jet. I think that's vital. You must know this or else you'll form the jet at first and keep using it even when the jet becomes ineffective. You must build enough experience as the practitioner

in order to know if you are using it properly. [Experience Needed for CureJet] So the CureJet is an exceptional device [Experience Needed for CureJet] but needs a lot of experience. Yes, that's important. We also built up insight as we continue to use it. But as we continue to use CureJet,

we will accumulate more insight. It actually hasn't been long since CureJet was released. So all of its users are beginners. But once we use it for 1-2 years, we'll gain more insight and we should be able to share this insight.

Since we have so many scar patients, even though it hasn't been long since its release, we've accumulated a lot of experience. We use it every day. On busy days, we each use it up to 10-20 times a day. Today we discussed acne scars and the newly released CureJet
