Hello, I’m Doctor Jaehyun Park at Dana Plastic Surgery. People who had hair transplant might have heard or been concerned about this problem. Today I will talk about shock loss. First of all, shock loss is divided into two categories. It could happen in the extraction area or the area it is implanted. I will talk about this separately.

So, the shock loss in the extraction area is divided into two categories. It could happen during the Follicular Unit Transplantation (FUT) or the Follicular Unit Extraction (FUE). I will talk about this separately and tell you about How we, at dana plastic surgery prevent shock loss.

I will talk about the shock loss using FUT method in the extraction area I repeatedly emphasized that scalp laxity and the size of the design is extremely important for the FUT method. For the design we have to decide where to make the vertical and horizontal cuts, where to extract from and to decide how to do the golden ratio design.

The doctor’s workmanship is of utmost importance and measuring the exact scalp laxity of the patient is the first thing to do. If the laxity is 15mm but we ended up designing 16mm we will have to forcibly pull the skin which will likely end up bursting or lead to a shock loss. If it’s 15mm, we normally leave about 2mm

and extract the rest. So if the scalp laxity is 15mm, the maximum you should cut is 13mm which will prevent shock loss and other problems to arise. Additionally, if you don’t have enough scalp laxity there is an exercise I recommend. If you click here you will be able to see the scalp

laxity exercise that I have uploaded previously. There are two ways to increase scalp laxity. The scalp laxity exercise and the H-lase injection. We are proud to say that in 2019, our hospital was the first to present an article about H-lase injection. You can check out the video of H-lase by clicking here.

So, the two ways of increasing scalp laxity are the scalp laxity exercise and the H-lase injection. It is the best way to prevent shock loss but it might still happen. It won’t prevent it 100%. If your hair follicle is thin and weak or if it’s your second operation it might occur with a higher possibility.

The suture method is also important. In the case of our hospital we suture two layers instead of one. We use absorbable suture for the layer that is under the hair follicle use nylon for the upper layer of the skin. By using this method, it prevents any possible damage that might be made in the grafts.

Using a suture method that will not decrease the blood circulation Is the most important aspect. This does not occur as often during the FUT But of course, it doesn’t prevent it 100%. Then what about FUE? During the FUE, there should be no restrictions in the blood circulation. I have uploaded a video about epinephrine which is used widely in the field

of hair transplant as a vasoconstrictor. If we use epinephrine more than the amount needed the blood vessels will shrink which will decrease the blood circulation. Therefore, you will need to know how much epinephrine to use. Secondly, using a punch that is the right size for the hair follicle is crucial. Using a bigger punch for easy extraction will widen the scar

which increase the probability of shock loss. Finding the exact depth of the hair follicle to punch is also crucial. Because punching in too deep will cause damage to the blood circulation that is under the hair follicle which will lead to shock loss. The density of the punch is also important.

I always emphasize that you have to use the donor area to decrease the density evenly, But if you target one area intensively, shock loss will occur in that area. These are the important factors for FUE and FUT method. Having a lot of experience will help you control these factors and will be able to put in the effort to prevent it from happening.

So what efforts can be made? In the case for the FUT method, I talked about scalp laxity And the H-lase injection. I also talked about scalp laxity exercise Which will increase the laxity. Considering the patients hair thickness and density to find the golden ratio is the key to prevent shock loss.

In the case for the FUE method I talked about using the right amount of epinephrine. And if you look at the area behind the ear, there is a small bump. this area is called the mastoid process. The skin around this area is extremely thin which means that blood circulation is poor.

Personally, avoiding this area while punching will prevent shock loss, which is just my personal experience. Having these little tricks will help prevent shock loss to the minimum. However, it won’t prevent it 100% of the time so there are situations where it might occur unpredictably. But it will reduce the chances significantly.

I talked about the shock loss in the extraction area. Now I will talk about the implanted area. The same goes for the implanted area. It is about how effectively you used epinephrine with the minimum quantity. And in many cases, the fundamentals are always the most important.

Hair transplant is a surgery where you cut into the skin thousands of times. Therefore, using the knife or needle to evenly cut the skin and to make a slit with a good technique is the most crucial part of this surgery. Having these fundamentals down will prevent shock loss in the implanted area. But these problems could still occur depending on the patient’s constitution.

Truthfully, shock loss is not something to worry about because you will recover in 3~4 months in most cases. So, if you are experiencing shock loss, how do you deal with it? First of all, go consult with the doctor that did the surgery. it will usually get better as time passes. I personally don’t use minoxidil but

In some cases it could be helpful or nonglutinous therapy which could give the hair follicles the nutrients it needs. We use these secondary treatments to regenerate the hair faster. In most cases it will recover about 99% of the time. Wait for about 3~4 months and most of the hair follicles in the implanted and extracted area will heal up.

There are a lot of people who think that FUE method leaves no scar. It leaves a lot more scar than the FUT method.
