Additionally, the chin juts forward significantly This is a typical prognathic jaw, with a sunken midface It gives the impression of a very flat face We aim to create a more three-dimensional appearance That’s why orthognathic surgery is necessary Orthognathic surgery repositions the jawbones to a normal alignment, enhancing facial depth

Since reducing the overall facial area is also necessary We will perform three facial contouring procedures simultaneously Maxillary (upper jaw) repositioning Mandibular (lower jaw) repositioning, shifting it upward and backward Plus, cheekbone reduction, jaw angle reduction, and chin reduction with advancement For non-specialists, it’s hard to visualize how these changes will look just by hearing about them

So I’ll roughly explain how we plan to modify the patient’s skeletal structure The face is actually a three-dimensional structure But since we’re explaining it on a two-dimensional screen It's easiest to illustrate using a side profile view This is the current side profile Compared to the normal female skeletal structure, it is highly developed

The bone thickness is quite substantial And the chin slightly protrudes forward Looking at the teeth alignment, the front teeth are significantly angled outward The area beside the nose appears sunken From here to here, the length is quite extended The chin is notably projected forward—technically referred to as prognathic

By repositioning both the upper and lower jaws backward and upward We will correct the protruding jaw and create a more contoured face Here’s how we’ll proceed In preparation for surgery, we go through a tracing process We mark key skeletal areas on the face Then we plan how to modify these areas—this process is commonly called "paper surgery" using tracing

I’ll show you this process and predict how the patient’s facial structure will change This is a printed version of the skeletal structure We first outline key skeletal points, including the positions of the front and molar teeth and jawbones There are standard reference lines we use for the skull We draw those lines And then trace the palatine bone, which forms the roof of the mouth

The upper jaw, which was protruding forward, is being rotated You can see how the upper jaw tilts downward as it rotates The front teeth tilt inward, while the molars elevate and shift upward and backward The palate also lifts further This moves the jaw closer to its normal position The front teeth shift slightly backward

And the molars move upward and backward Next, we reposition the lower jaw by aligning it with the newly adjusted red upper jaw line This results in the following transformation This is the change after both upper and lower jaw surgeries The lower teeth retract slightly, and the prognathic jaw is corrected The lower molars align with the repositioned upper molars

The chin’s position is adjusted based on the ANS (Anterior Nasal Spine) reference point The upper and lower jaws rotate into their new positions Since the lower jaw shifts further back than the upper jaw This much jawline retraction resolves the prognathism At the same time, the lower jawline moves upward Resulting in a reduction in overall facial length

The excessive vertical length also decreases Even these changes alone bring a significant improvement To further enhance facial slimming, the third step involves modifying the red jawline and chin area In this third step, we perform contouring surgery, including square jaw reduction and chin reduction The overall length is shortened Then, we adjust the square jaw accordingly

The final result follows this black line We cut the middle portion of the chin twice The removed bone segment is discarded, and the lower portion is lifted and slightly advanced before fixation A slight advancement helps refine the side profile Finally, we refine the square jaw contour to match the adjusted height Using this plan, we take an impression of the patient’s teeth
