I see many different types of patients in facial bone contouring consultations. Some may vaguely say "my face is too big and wide," or "I want to be pretty," not even knowing what facial bone contouring surgery is. And some say "I want to have zygoma reduction without any bone removal," or "I am afraid of genioplasty that includes T-osteotomy becuase it breaks into too many pieces of bones,"

giving specific plan of the surgery all the way through. From my point of view having to give a diagnosis, neither situation is advisable. For those who have too little information, I have to spend less time on talking about really significant information, having to explain from a to z. On the other hand, for those who have too much information, I have to spend a lot of time

because they tend to have mistaken idea about the surgery. So I want to talk about things that you should consider before having a consultation with me. "Things to know before coming to the facial bone contouring consultation" First of all, you should think about what kind of face line you prefer or like. You should give a definite and specific thought about what kind of changes you want to bring to your face.

Beauty, itself, is a very subjective concept. You might have experienced some kind of situation where you talked about how pretty or beautiful some celebs are, and your friends did not agree with you, right? On the other hand, they said one was very handsome and good-looking, yet you didn't find him or her attractive. The concept gets more complex if we talk about attractiveness over

how pretty, how beautiful, or how handsome they are. So, having a very specific standard is very important. What I suggest is you bring many photos of celebrities or even edited ones of yourself. The more photos or information you bring and give to me, the clearer idea I get about what you want. Don't be too shy or embarrassed about bringing photos of celebrities. You may think "Isn't she too pretty to be my goal?",

or "Is this face line fit for me?" At this point, you shouldn't think about whether it is possible or suitable for you. When we have a consultaion together, I will tell you about those details. By talking thoroughly about the goals and desires, narrowing down the gap between the doctor and patient is the most significant. We should be spending the most time there.

"Terms to know about facial bone contouring surgery." Well, I want to tell you about what facial bone contouring surgery is. Looking at this photo, we can divide outer face line into three sections. The first section is adjusted by zygoma reduction, the second section is adjusted by mandible contouring operation, and the last section is adjusted by genioplasty.

And when we adjust all these three areas, which are zygomatic, madibular, and chin bone, we call it total contouring operation. "The differences between facial bone contouring surgery and orthognathic surgery" You may wonder about the differences between facial bone contouring surgery and orthognathic surgery. Some of you think they are same,

and some of you think orthognathic surgery is more superior and more complex concept than facial bone contouring surgery. Two of them are actually completely different surgeries. Orthognathic surgery is designed to correct mouth protrusion or pronathism, which includes moving of teeth. However, facial bone contouring surgery does not involve any movement of teeth. "Mandibular canal and plate fixtion"

If you research or consult a lot about facial contouring surgery, you often hear these two words: mandibular canal and plate fixation. There are two holes in the mandibular canal, front and back. They are called mental foramen and mandibular foramen. Inferior alveolar nerve from brain comes out and goes through the mandibular canal.

If facial bone is cut beyond the canal, it damages the canal and causes sensory disturbances in teeth and lips. Therefore, the bone should not be cut beyond the canal; it should be the limitation line of the facial bone surgery. Plate fixation is used when we have to fix bone pieces with screws and plates that are made with titanium. These titanium screws and plates do not have magnetic forces,

so they don't cause any problems at airport security or in MRI, etc. "Surgery method" In zygoma reduction, there are two main ways to operate. One is called "ostectomy," where a partial bone is removed and remaining bone is relocated. In this case, zygomatic bone is rotated or pushed up, so the volume of zygomatic bone is reduced.

The other is called "osteomy," where facial bone is seperated and relocated to the back side without any bone removal. These two techniques are used in difference indications, but I tend to use both in the surgery. For madible contouring operation, I shorten the angle through curved ostectomy and shorten the width through corticectomy or other ways. For genioplasty, there are also two main ways to operate.

One is called "T-osteotomy" and the other is called "inverted V-osteotomy" T-osteotomy is specialized to shorten the width of the chin, and inverted V-osteotomy is specialized to shorten the length. There are many other techniques to operate, but they are modifications of these two methods.

So I use one of the methods depending on the indication or use them both if needed to give you the best result. Again, you should not be spending much time thinking about the methods of the surgery, I will give you the solution that fits you and brings out the best outcome for you.
