다시 눈 이야기로 포스팅합니다.
오늘은 조금 특별한 눈수술 케이스에 대해서 말씀드리고자 합니다.
(Today, I would like to tell you about a little special eye surgery case.)
한국의 로컬 성형외과의원들은 보통 내국인들과 중국인들을 타겟으로 마케팅을 하고 주로 수술을 하는 외국인들은 같은 아시아계열인 동남아나 중국, 일본인들인데요.
(Local plastic surgeons in Korea usually market to Koreans and Chinese, and foreigners who mainly perform surgeries are Asians, such as Southeast Asians, Chinese, and Japanese.)
세브란스병원은 특성상 중동이나 아예 먼 해외에서 오시는 분들도 많습니다. 병원에 아예 외국인 전담팀도 따로 있고 통역사들도 따로 있습니다. 간호사님들 중에서 그쪽 언어를 잘 하시는 분이 국제진료소에서 데려와주셔서 통역도 해주시는 경우도 많습니다.
(Due to the nature of Severance Hospital, there are many people who come from the Middle East or from far abroad. There is a separate team dedicated to foreigners in the hospital, and there are separate interpreters. There are many cases where nurses who speak the language well come from the International Clinic and translate for the foreign patients who visit our hospital.)
이 때까지 외래에 있어보면, 일본, 중국, 프랑스, 미국 등 외국인도 매우 많이 의뢰되어 오거나 상담받으로 오시는 분들이 종종 있었습니다.
(Up to this point, when I was at my outpatient clinic every Wednesday, there were often a lot of
foreigners from Japan, China, France, and the United States who came to our clinic for various consultations)
몇 달 전 프랑스 국적의 백인 여성이 원래 두꺼웠던 본래의 쌍꺼풀이 피부가 처지며 얇아지고 눈 크기도 작아진 것 같고 무거워진 것 같다는 내용으로 상담하러 오셨습니다. 얼굴에있는 여러 큰 튀어나온 점들도 레이저로 제거되지 않는다고 수술로 흉이 안남게 제거해달라고 하셨어요. 이 케이스 경과에 대해 말씀드리고자 합니다.
(A few months ago, a Caucasian woman of French nationality came to see me for a consultation concerning her eyes and hairy nevi, complaining that her double eyelids, which were originally
thick, had drooped and thinned skin, and that her eyes had become smaller and heavier than before when she was 20's. Many large protruding hairy nevi on the face were also said to be removed
with lasers, so she asked me to remove them without scarring. I would like to tell you about the progress of this case.)

38세 여성, 서양 백인 여성 상하안검성형술 및 눈매교정, 수술적 점 제거 (Upper and lower blepharoplasty with ptosis correction, Hairy nevus excision, PreOP)
너무 이쁘시죠? 역시 백인 여성이라 그런지 얼굴도 작고 하얀 피부에 눈도 선천적으로 매우 크고 몽고주름도 없습니다. 선천적인 아웃라인의 high fold가 매우 잘 어울리시네요.
(She is so pretty. After all, because she is a Caucasian woman, her face is small, white skin,
and her eyes are naturally very large, and there are no Mongolian folds. The high fold of the innate outline suits her very well, of course.)
사실 성형외과학 자체가 미국에서 시작한 의학이고, 전 세계적으로 요즘 통일된 교과서가 미국 성형외과 교과서인 Neligan Plastic Surgery인데, 그 교과서에 실린 여성분 같은 느낌이 들었습니다. (In fact, plastic surgery itself is medicine that started in the United States, and the textbook that has been unified worldwide these days is Neligan Plastic Surgery, an American plastic surgery textbook, and I felt like she is a woman introduced as a case in that textbook.)
프랑스 분이시지만, 영어를 할 줄 아셔서 상담은 전부 영어로 진행했습니다. 나이가 들면서 피부가 늘어져지고 눈 뜨는 힘이 약해지는 것 같고, 아래 눈꺼풀도 주름이 많이 생겨 어두워지는 것 같다는 말씀을 하시네요. 지금도 크신데 젊었을 때는 훨씬 눈을 더 잘떠서 시원하고 밝게 보이고 쌍꺼풀도 더 두꺼웠다고 하십니다. (Although she is French, she can speak English, so all consultations were conducted in English. She said that as she gets older, her skin sags and her ability to open your eyes weakens, and her lower eyelids also get darker due to wrinkles. Her eyes are still large, but when she was younger,
she opened her eyes much better, so he looked cool and bright, and she said her double
eyelids were thicker.)
젊었을 때와 같이 쌍꺼풀이 높고 진하게 잡히도록, 그리고 눈이 더 커지고 무거운 느낌이 들지 않게 해달라고 하셨습니다. 그래서 플랜은 피부를 약간 잘라내고 쌍꺼풀 부착지점을 지금 잡힌 곳보다 조금 더 어울릴 만한 곳으로 높이는 쌍꺼풀수술과 눈매교정, 그리고 하안검 주름과 눈물고랑을 없앨 목적의 하안검성형술을 하기로 계획했습니다. (She asked me to make her double eyelids look high and thick like when she was young,
and not make her eyes feel heavier. So, the plan was to do double eyelid surgery to slightly cut the skin and raise the attachment point of the double eyelid to a more suitable place than the current one, correcting the eyes, and lower blepharoplasty to remove wrinkles and tear troughs on the lower eyelid.)
수술 과정

실제 안검하수가 있는 분이 아니라 levator detachment후 advancement나 reinsertion이 필요하지는 않았고, 가단하게 tarsus를 찾아 그 보다 얕에 놓여있는 levator aponeurosis를 아래 모식도대로 접어주는 plication술기만 해도 이런 젊은 여성들은 눈이 훨씬 생기있게 잘 떠지게되고, 결국 눈이 커보이게 됩니다. (She doesn't have actual blepharoptosis, so advancement or reinsertion after levator detachment was not
required,and the plication technique of finding the tarsus and folding the levator aponeurosis shallower than it in the schematic diagram below makes this young women much more lively. It opens well, and eventually the eyes look larger.)
하안검 수술은 아래와 같이 skin을 잘라내는 transcutaneous apporoach로 했는데, 실제로는 septum 을 arcus marginalis에 걸어주는 septal reinforcement도 예방적으로 같이 해주었습니다. (The lower eyelid surgery was performed as a transcutaneous aporoach that cuts the skin as shown below, but in fact, septal reinforcement by anchoring the septum to the arcus marginalis for a prophylactic perpose)

수술 5일차 실밥을 뽑았습니다. 저는 보통 7일차에 뽑는 편인데,이 경우는 Rt. cheek nasolabial segment와 Rt. side upper lip 의 nevus excision한 부분도 실밥을 뽑을 겸 해서 일찍 뽑았습니다. 백인은 상대적으로 흉도 덜남고 흉터나 상처로부터 다른 인종들보다 자유로운 부분이 많아 술기가 제대로 됐다면 흉터에는 크게 신경 쓰지 않았습니다. (The stitches were removed on the 5th day of surgery. I usually remove them on the 7th day, but in this case Rt. cheek nasolabial segment and Rt. The nevus excision part of the side upper lip was also pulled out early to double as a stitch removal. Caucasians are relatively less scarring and are free from scars than other races, so if the surgery was done properly, I do not pay much attention to scars after Caucasian patients.)
그리고 서양인들은 쌍꺼풀 고정(double fold fixation)할 때 저는 가장 오래가는 녹는실(PDS)을 씁니다. 일반 우리나라 사람들을 포함한 아시아인들 수술할 때는 녹지 않는 실(Nylon)을 사용하고요. 그리고 이렇게 deep-set eye에서 fold를 확실하고 단단하게 고정시키면서 high fold를 맞추고 싶을 때는 septal fixation이나 SAJT fixation 이 아닌 안검판(tarsus)에 완전하게 고정해주는 TAO triple fixation (타오 삼점 고정)을 사용합니다. 조인창 원장님이 가장 많이 쓰시는 방법이죠. 조인창 원장님은 보통 우리나라 사람들 수술할 때도 TAO fixation을 쓰시는데, 가장 잘 풀리지 않는 장점이 있는 방법이긴 합니다. 그리고 이 케이스처럼 눈매교정을 해줘야 하는 경우 tarsus를 노출시켜 levator aponeurosis를 plication하던지 잘라서 당겨주던지 간에 tarsus에 어쨌든 걸어줘야 하죠. 그러니 TAO가 맞는 방법 같습니다. (And when performing double fold fixation to Caucasians, I use the longest-lasting dissolving thread (PDS). Non-melting threads (Nylon) are used
when performing surgeries on Asians, including Koreans. And when I want to adjust the high fold
while fixing the fold securely and firmly in this deep-set eye, I use TAO triple fixation (TAO three-point fixation), which is completely fixed to the tarsus, not septal fixation or SAJT fixation. This is the method Dr. In-Chang Cho uses the most. Dr. Cho In- chang usually uses TAO fixation when
performing surgeries for Koreans, but it is a method that has the advantage of being the most difficult to untied. And if you need eye correction like in this case, you have to hang it on the tarsus anyway, either by exposing the tarsus and plicating the levator aponeurosis or by cutting and pulling it. So TAO seems to be the suitable way in this case.)

위 원리처럼 쌍꺼풀을 고정을 직접 tarsal plate에 물어주는게 원리입니다. (As in the principle above, the principle is to apply the fixation of the double eyelid directly to the tarsal plate.)
아직 5일째라 부기로 인해 결과 예측이 정확히 안되긴 하는데 고객분이 원하는 대로 out-fold가 잡혔고, 눈 감았을 때 보면 MPL에 높게 10mm이상 잘 잡혀 있긴 합니다. 이제 부기만 빠지길 기다리면 되겠습니다!
(It is still on the 5th day, so it is difficult to accurately predict the result due to swelling, but the out-fold was created as the customer wanted, and when she closed her eyes, the MPL was well over 10mm high. Now all she has to do is wait for the swelling to go down!)

수술 후 2주 째 사진입니다. 사실 전 눈수술 2주 째는 굳이 따로 보지 않는데, 몸의 다른 부분에도 많은 hairy nevus를 같이 절제해줬기 때문에 몸의 실밥을 뽑으러 오셨습니다. 그래서 겸사겸사 눈도 봐달라 하셔서 사진을 남기게 됐습니다. 왼쪽 하안검 부분에 Grade I 정도의 경미한 외반이 있네요. 2주에 이 정도면 시간 지나면 좋아집니다. 오히려 적절하게 딱 맞게 피부를 잘라 잘 된거라 생각합니다.
(These pictures are taken 2 weeks after surgery. In fact, I didn't bother to see it separately
for the second week of eye surgery, but she came to have the stitches removed because many hairy nevi were removed from other parts of the body as well. So, she asked me to take a look at her eyes, so I took picture for the comparison of the progress. There is a mild grade I lower lid eversion in the left lower eyelid area. If this is enough for 2 weeks, it will get better with time.
Rather, I think it did well by cutting the skin to fit properly.)

수술 후 1달 째 사진입니다. 흉터도 1달 째 이정도라면 나중에는 아예 티가 안날 정도로 옅어질 겁니다. 무엇보다 오른쪽 뺨과 입술 위에 제거한 점 흉터가 거의 안남아 있어 좋네요. 몇 개월 더 지나면 아예 흔적도 안남을 겁니다.
(One month after surgery. If the scar is like this for 1 month, it will fade to the point where it is barely noticeable later on. Most of all, it's nice that there are almost no scars left on the right cheek and lip. In a few more months there will be no trace of it at all.)
이런 deep-set eye 에 epicanthus가 없는 눈은 자연스럽게 해주려고 오히려 낮게 잡으면 더 어색합니다. 전형적인 계란형 얼굴에 콧대가 높고 눈썹 바로 아래 뼈보다 훨씬 깊이 위치한 안구라면 쌍꺼풀 높이를 10mm이상 높게 잡아주는 게 3개월 후 길게 봤을 때 더 자연스럽고 시원한 인상을 줍니다. (Eyes without epicanthus in these deep-set eyes are more awkward if held rather low to make them natural. If you have a typical egg-shaped face with a high nose bridge and eyeballs
located much deeper than the bone just below the eyebrows, raising the height of the double eyelids by at least 10mm will give a more natural and cool impression when viewed long-term after 3 months.)
아직 한 달 째라 붉은기가 있긴한데, 동양인에 비해서는 그 quality가 훨씬 좋긴하네요. 몇 달 후 경과가 어떨지 궁금합니다 개인적으로는 한 달 째 이 정도의 높이가 딱 어울리고 눈에 어울린다고 생각하는데, 지금보다 조금 더 높이는 낮아져서 계속 유지될 겁니다. 조금 더 자연스러워 지겠네요. (It's been a month since it's still red, but the quality is much better than Asians. I wonder what the progress will be after a few months. Personally, I think that this height suits the eyes just right for a month, but a little higher than now will be lowered and maintained. Maybe a little more natural.)

완전히 부기가 다 빠진 건 아닙니다.
남은 2개월에 거쳐 서서히 높이도 지금보다는 조금 더 낮아지면서 자연스러워 질겁니다.

수술 전과 비교하여 훨씬 밝아지고 서양인에 어울리는 이쁘고 높은 자연스러운 아웃라인 쌍꺼풀과 midcheek이 밝게 잘 자리 잡혔죠? 이제 붉은 흉터만 사라지면되는데... 최소 몇 개월은 더 걸릴겁니다. (It's much brighter, and the pretty, high, natural outline double eyelids and midcheeks that suit westerners are brightly settled, right? Now, only the red scars need to disappear... It will take at least a few more months.)

수술 전 피부가 다소 늘어져있고, 눈을 이전만큼 크고 밝게 못뜹니다. 눈 아래쪽 피부도 늘어져 어두운 느낌을 줍니다. (Before the surgery, her skin is somewhat sagging, and her eyes are not as large and bright as before. The skin under the eyes also stretches, giving a darker look.)

(One month after the surgery, the scars haven't disappeared yet, but the saggy skin and dark areas under the eyes have been resolved, and the eyes are as big and bright as before. It is much brighter and gives a younger feel.)
