are actually ideal for Koreans. Is 300 shots really enough, or do we need to go higher, like 800 or 1,000 shots? We'll be testing it out on our own faces. Oh, I see. Walking in, all the doctors are getting treated right now. But Doc, aren't you going to apply any numbing cream?
Real men don't use numbing cream. Your jaw is going to feel like it's shattering... Wait, hold on. "The Real Man's Ulthera." Today we have 300, 300... 600, 800 shots. No one wants to do it without cream. Why is that? It doesn't even hurt that much. [Laughter]
It hurts! It's totally fine to get it. We're real men, men don't need numbing cream. 40... Some clinics recommend 300 shots, while others suggest 800 or 900 shots. Why do recommendations vary so much by clinic?
Ultimately, it comes down to cost. The official Ulthera guidelines suggest 1,250 shots in one session, but realistically, almost no one pays for 1,250 shots. And that's because of the price. It's just too expensive. For 1,250 shots, based on our clinic's rates, it would cost around 5 million KRW.
5 million KRW? Yes. But that's not realistic for most people. It's too much of a hurdle. So, if you have the budget and don't mind spending that much, you can absolutely go for it. Otherwise, people look for the most cost-effective shot count.
Then what shot count do you recommend as the perfect compromise between cost and results? For the jawline and lower face, which are the most common areas, around 400 shots is standard. For a full face, 600 shots is generally the most popular. But lately, adding the 1.5mm transducer tip
to improve the outer layer of the skin and going up to 800 shots has become the new benchmark for a full face. How are you feeling, Doc? Yes. How's the pain? Is it bearable?
[Laughter] I'm fine. You brought this plushie because you like it, right? It's yours. By the way, is anyone actually satisfied with a lower shot count? Well, it can feel a bit lacking. Some people get just 200 shots for the jawline. But it leaves you wanting more,
because everyone knows that more shots mean better results. Again, it's a matter of cost. Honestly, there's only one way for Ulthera to get cheaper. The manufacturing company needs to lower their prices. [Laughter] Let me show you... If you could make me the CEO... [Laughter]
On the flip side, it's not like more shots are automatically better. Blindly getting a high number of shots won't help. You have to allocate the right number of shots to the right areas. They say it's fine to go up to the official "Gold Standard" protocol, but if you keep firing shots continuously just at the jawline, it will cause volume loss in the cheeks and ruin the facial contour.
If you distribute them evenly across the face, using the correct energy levels and depths for each area, you will see fantastic results. If you were treating your own family, how many shots would you give them? If we're going for true cost-efficiency, like I mentioned, 800 shots for the full face might feel like too much.
If they say, "That's a bit too many, should I just do 400?" Then 400 shots to focus on the lower face, or anywhere between 600 to 800 shots for a full face. That's the ideal approach. What if you were getting it yourself right now? My under-eye area has been losing volume lately.
And my face is sagging a lot. I'm really starting to show my age. It's definitely sagging. Yeah, it is. I can really feel it. Personally, I'd go with 600 shots. 600 shots? Yes, 600 shots for me.
600 shots? Hmm. Whenever my wife comes across my Shorts or Reels on her feed, she gets genuinely startled. Why? She asks why I look so much older on camera. [Laughter] [Music]
Does treating the brow area make the eyes look bigger? It actually makes opening your eyes feel a lot lighter. We call it "Eye Ulthera," where we target the brow line. It genuinely eases the effort of opening your eyes and helps smooth out fine wrinkles. We've hit 300 shots. Should we do a bit more?
Let's stop here. Thank you. How many shots did you end up getting? I got 300 shots. 300 shots? How was it? Did it hurt? It wasn't painfully unbearable. And you didn't even use any numbing cream!
Who has the time to apply it, wait, and then wipe it off? My mind is completely wiped right now. After getting it, it doesn't swell that much. Yeah, it looks fine. But if you concentrate it heavily on one area, the face can puff up quite a bit the next day.
Ah, I see. I don't think mine will be that bad though. Even if it swells, it usually goes down in a day or two. Actually, severe swelling can last for about a week. So if you absolutely cannot have swelling, you need to distribute the shots evenly across the entire face. But if you're okay with swelling and prioritize results,
focusing the shots on the jawline works best, though it will swell. So it takes about a week to subside? Yes, it takes about a week. There's a rumor that too few shots won't show any effect. But on the flip side, people say too many shots can hollow out your face. That's why you shouldn't overlap too many shots in a single spot.
That can cause indentation or volume loss in that area. Also, we need to prevent the internal tissue temperature from rising too high. If you spread the shots out evenly, you can theoretically safely do over 1,000 shots. Problems only happen when you blast all the shots into one specific spot. People also say Ulthera causes cheek hollowing. Does it really?
It can cause fat volume loss, specifically around this area. That's why we usually map it out beforehandβthe markings faded a bit nowβ but we mark the face to avoid those areas. As long as it's properly designed first, you don't need to worry about cheek hollowing. Some middle-aged women already have significant volume loss there, but we assess and customize the design before starting, so it's perfectly safe.
Can you give a definitive shot count for the general public? If you don't have much facial fat and just want to fix sagging in the lower face, around 400 shots is recommended. If you want an overall lifting effect for the full face, 600 shots. And if you have more facial fat and want a dramatic change, go for 800 shots. People often get both Ulthera and Thermage. Which one hurts more?
Which one did you find more painful? I think Ulthera is much easier to tolerate than Thermage. So Thermage is way more painful? Yes. Out of 10 people, 8 or 9 will say Thermage hurts more. Thermage feels much hotter. Imagine a hair stylist holding a scorching hot hair dryer to your skin for too long.
It feels like that. Like a hair dryer left in one spot. Has a patient ever come back a few days after Ulthera, unhappy with the results, and demanded a redo? No one has ever asked for a redo, but I've had occasional complaints about the swelling. Areas close to the jawbone tend to swell more.
It's rare, but those prone to swelling can stay puffy for 1 to 2 weeks. But it goes away after 1 or 2 weeks, right? Of course, it goes away and looks even better. It usually doesn't even swell that much. In fact, some people who got 300 shots come back asking for a few more. Like, asking for extra shots as a free service? Not yet. But there is a quirky thing about Ulthera.
Most lifting devices trigger an alarm exactly at 300 shots. But this machine doesn't have that. So while chatting with a patient, I sometimes accidentally fire 10 more shots without realizing. So I do end up giving a couple extra shots here and there. Should I fake being in pain then? Are you really not in pain?
I'm fine with this... Wait a second, it actually hurts a lot. No, it doesn't. It doesn't hurt. Honestly, anyone watching can tell you're in pain. No, no, no. Oh, hold on. My sneeze... my sneeze is proof. Yeah, my sneeze... I held it in. Let's try again.
Man, that spot is weirdly painful. [Laughter] It's weird. Maybe you're just tired today. Yeah, I didn't sleep well last night. Right? I think it's the lack of sleep... Whoa, wait. It feels like my teeth are going to shatter. Why is it like this?
Man, I feel like I lost a tooth. Can we keep going? Please target the area above my nasolabial folds too. Will that smooth them out? Reducing the fat above the folds makes a difference... Wait, I don't think I can do this. Hold on. [Laughter]
Just two more shots left. Alright, let's do it. Wow, the upper face is more painful than I thought. Yes. So how long do the results last after one session? For general maintenance, people get it once a year. If you want more noticeable improvement, up to twice a yearβabout every six months. So you can expect it to last between six months to a year.
Some videos suggest getting it every three months. And I heard a celebrity named Kwak Tube gets Ulthera once a month. Once a month? Yes, apparently he said that in an interview. I'm not sure that's entirely necessary. It takes about a month after the procedure for collagen to actually start forming.
Getting it every month feels a bit excessive. Setting aside whether the skin can take it, the cost would be overwhelming. It's already expensive, so every month is wild. What area are patients usually most obsessed with? The most common concern is the jowl area right here. Sagging jowls and a soft, undefined jawline.
So the jawline and jowls are the most heavily requested, along with the upper nasolabial folds we're doing now, and the cheekbones. What is the typical age group for Ulthera patients? It's most popular among people in their 30s and 40s. There's some debate on the ideal age to start. It used to be said that those in their 20s don't need it,
and that you should start in your 30s when collagen depletion begins. But surprisingly, a lot of people in their 20s get it these days. I mean, it doesn't hurt to do it early. But if your face hasn't started sagging yet, I don't see a pressing need to get it at such a young age. In reality, I'd say 9 out of 10 patients are highly satisfied. Of course, satisfaction varies from person to person,
but almost no one claims it had absolutely no effect. However, if your expectations are sky-high, you might be disappointed. It's not going to pull your face back as drastically as a manual facelift. During consultations, some patients pull their skin up with their hands and ask, "Will it lift this much?" It won't do that. Even surgery can't achieve that exact look.
But the before-and-after difference is definitely noticeable. Those who expect a dramatic surgical pull might be less satisfied, but overall, 9 out of 10 people are happy with it. Thank you. Did you notice any difference in pain as the shot count went up? The pain felt pretty consistent throughout. So there's no reason to lower the shot count just out of fear of pain. Exactly. Pain doesn't scale up with the number of shots. So you shouldn't feel pressured about getting a high shot count because of pain. Whether you get 300, 600, or 800 shots, the pain level stays the same.

Come on, cut it out. Seriously. How did it feel getting Ulthera today? It was totally manageable. [Laughter] Awesome. And how long did you say the effects last? The real visible improvement shows up about a month later. Rather than an instant change, collagen builds up over the month, tightening the skin.
The peak results last about six months to a year. If you want to be proactive about lifting, go every six months. If you view it as a long-term savings account for your youth, once a year is perfect. But seriously, I'm not just saying thisβit already feels tighter. It's an amazing treatment, just pricey. Can we just do 100 shots as a trial?
Some places offer 100-shot trial programs, but honestly, 100 shots will leave you feeling a bit disappointed. You need at least 300 to 400 shots to see proper results. 600 shots is even better, and 800 shots is the best.
