Hello, this is Chamin Plastic Surgery.
Today, I’d like to explain a case involving a patient who underwent complete removal of the nasal bridge and tip, and had a protruding Medpor implant removed.
Complete removal of the nose (silicone, Medpor removal)
Chamin Plastic Surgery

Preoperative history
About 10 years ago, after having rhinoplasty with silicone and Medpor at another hospital,
recently the tip of the nose repeatedly became red and then improved,
and after pus-like discharge appeared and burst around the columella, the skin became indented, leading the patient to visit Chamin Plastic Surgery.
Even from the outside,
an artificial cartilage, presumed to be Medpor, had broken through the skin,
making the skin look sunken.

Condition of the nose before surgery
-
Currently, the columellar artificial cartilage is protruding
-
The columellar skin is defective
-
Inflammation is judged to be progressing
The situation was suspected to involve inflammation, and there was a very high possibility that the skin had grown along the Medpor, so urgent complete removal surgery of the nose was needed.

Nasal bridge: silicone removed
Nasal tip: Medpor removed, nasal tip cartilage repositioning
This is what the removed silicone and Medpor looked like.
Approximately 3 weeks after complete nasal removal

Although it is still the period when the incision line and swelling remain,
after removing the protruding Medpor and removing the silicone in the nasal bridge,
you can see that it is healing well.
Now, here is a comparison before and after complete nasal removal.
Before surgery and about 3 weeks after surgery

Before surgery / 3 weeks after
From the front, the protruding Medpor in the columella and nasal tip area has been neatly removed.
The indented area may later require a skin graft or a second revision surgery depending on the progress,
but we were able to confirm that the patient’s nasal condition had improved greatly at this point.

Before surgery / 3 weeks after
As the silicone in the nasal bridge and the Medpor in the nasal tip were removed,
the height became lower, but after the removal surgery, the nasal tip cartilage was repositioned
back to the original shape and position of the nasal tip cartilage,
so the tip did not turn up or collapse and was maintained naturally.

Before surgery / 3 weeks after
Since it is still the period when bruising and swelling remain after surgery,
the redness is quite visible, but the protruding area and
the indented columellar skin area
are healing well.

Before surgery / 3 weeks after
From the side as well, the height is lower, but the line is being maintained naturally.
After complete nasal removal, patients often ask whether the nose will turn up or collapse,
but because the surgical method and recovery process can vary depending on the current materials and condition inside the nose,
if you are considering complete removal, it is most accurate to visit the clinic, get a 3D-CT scan, and receive a consultation with the doctor.

Before surgery / 2 weeks after
The nose that had tilted to one side due to inflammation and the implant
after the implant and inserted nasal tip cartilage were removed,
gradually returned close to the appearance of the patient’s original nose.

Before surgery / 2 weeks after
The overly high and blunt-looking nasal tip now has a naturally formed line.
Complete nasal removal surgery is not simply a procedure to remove inserted implants and cartilage.
It is a surgery that requires appropriate removal and repositioning so that damage to normal tissue is minimized and the original shape of the patient’s nose can be restored.

Therefore, it is most important to find a doctor with extensive experience in removal surgery
and strong surgical skills.
If you are worried about revision rhinoplasty or removal surgery,
Chamin will be with you.
