Nose Reconstruction Surgery (Saddle Nose, Pinched Nose, Depression, Contracture) Extreme Nose Case

Today, I’d like to explain the case of a patient who came to Cha Min Plastic Surgery six months after having silicone removed at another hospital following nose surgery, after the silicone had protruded outside the nose.

The patient previously had rhinoplasty using silicone and septal cartilage.
Over time, the silicone protruded at the nasal tip, so the implant was removed. However, the inflammation continued, and the nose became contracted, pinched, and upturned, showing the appearance of an inflamed contracted nose.


<Condition of the nose before surgery>
- Contracted nose due to inflammation
- Shortened and upturned nasal tip
- Saddle nose deformity caused by weakened septal cartilage
- Pinched nostrils
- Depression at the nasal tip (scar)

The patient said septal cartilage had been used in a previous rhinoplasty, but because too much of the septum had been harvested, the strength of the septal cartilage had weakened. On the nose CT, we could confirm a saddle nose deformity with the bridge collapsed inward.
Because using silicone again in an inflamed nose makes the chance of recurrent inflammation very high, the nose reconstruction surgery was performed using the following materials:
Bridge: AlloDerm
Tip: Autologous costal cartilage + AlloDerm
Depressed contracted area: Autologous dermis

These are the appearances before and two months after nose reconstruction surgery.
The severely pinched area at the nasal tip has been well corrected, right?
The bridge height has also been raised naturally. At the two-month mark, there is still a lot of residual swelling, and it is best to observe swelling for at least 6 months to 1 year.


It can be seen that the area where the silicone protruded, the wound area, and the pinched nostril area have all returned to a rounder shape.


The short, thick-looking nose was reconstructed into a natural line, and both the bridge and nasal tip height were shaped to suit the face and overall impression.
After rhinoplasty, inflammation and contracture often cause the nose to shrink and be accompanied by pinching, so if inflammation or any abnormal symptoms occur in the nose, prompt treatment is the most important thing.

In the preoperative selfie photos as well, the pinching and depression at the nasal tip were clearly visible, and these were well corrected after nose reconstruction surgery.


The patient recovered well without major swelling, and their facial impression changed a lot after reconstruction.
