As the anesthesia begins to wear off, you may start to feel a throbbing pain little by little.

"How long is this pain going to last?"

"I have to go to work or school tomorrow—

will my face swell up like a hamster?"

These worries may come first.

Wisdom tooth extraction involves making an incision in the gum

and splitting the tooth to remove it...

It is, in effect, a minor surgery performed inside the mouth.

For that reason, pain and swelling are natural parts

of the healing process,

but how you manage this period

can make a huge difference in recovery speed and quality of life.

Today, we’ll talk about how long pain and swelling continue after wisdom tooth extraction,

and how to care for them to reduce swelling more quickly.

Wisdom tooth extraction pain: how long does it last?

The pattern of pain varies depending on how deeply impacted the tooth is

and how difficult the procedure was,

but the general recovery process is as follows.

The most painful time is right after the anesthesia wears off (3–5 hours later).

As the anesthesia fades, sensation returns

and throbbing pain begins.

This can be the hardest part,

so a helpful tip for reducing pain is to take the prescribed painkiller

before the anesthesia wears off (right after the extraction).

✅ Day 1–2 after extraction (peak period)

This is the time when an acute inflammatory response occurs.

Pain may continue, and especially when you lie down at night to sleep,

blood flow can shift toward the head and cause more throbbing.

You should take the prescribed medication on time.

✅ Day 3–5 after extraction (settling period)

This is when the pain gradually starts to subside.

The throbbing decreases, but a dull, heavy feeling may remain.

If three days have passed and the pain has not eased,

or if it suddenly gets worse, you should suspect dry socket or infection,

so contact the dental clinic immediately.

✅ Day 7 after extraction (stitch removal)

Once the stitches are removed, the foreign-body sensation disappears and it feels much more comfortable.

From this point on, there is almost no disruption to daily life.

Your cheek may puff up as if you were holding a candy in your mouth,

and in severe cases, bruising can extend below the jaw.

This is a natural healing response that happens as blood and lymph fluid gather at the surgical site

to repair damaged tissue.

Starting around day 3–4, with day 3 as the turning point,

swelling gradually begins to go down,

and it usually takes about 1 week, or up to 2 weeks,

for the remaining swelling to completely disappear

and for your face to return to its previous shape.

How to minimize swelling and pain

Is there a way to minimize swelling and pain?

Is time simply the only medicine?

Cold compresses only for up to 48 hours (most important)

For the first two days after extraction, ice packs are essential.

Cold temperatures constrict blood vessels,

preventing swelling from getting worse,

and dull the sensation to reduce pain.

If your heart is positioned lower than your head,

blood can rush toward the face and make swelling and pain worse.

When you sleep, use one extra pillow than usual,

or keep your upper body slightly elevated;

this is effective for reducing morning swelling.

For the first 1–2 days after extraction, it is best to eat porridge, ice cream,

and yogurt.

The pressure (negative pressure) created when sucking through a straw

can dislodge the blood clot that helps stop bleeding,

leading to dry socket, where the bone is exposed.

Smoking is the same,

and the hot smoke irritates the wound,

so please refrain from smoking for at least one week!
