I am often asked this question when treating gum-related conditions.

When patients come in for scaling and are advised to receive gum treatment, it is understandable to feel as though they are being asked to undergo the same thing twice or to wonder whether treatment is being unnecessarily increased.

To get straight to the point, although scaling and gum treatment have similar-sounding names, they serve different purposes.

Today, I will explain how the two differ and why scaling alone may not be enough in some cases.

Scaling Is “Cleaning,” While Gum Treatment Is “Treatment”

Let’s start with scaling.

Scaling is a procedure that removes tartar attached to the tooth surfaces and the border between the teeth and gums.

It mainly removes tartar above the gumline, in areas that are visible. Receiving it regularly is a good way to help prevent gum disease.

The problem arises when inflammation has already progressed deep inside the gums.

If tartar has penetrated below the gums toward the tooth roots rather than remaining above the gums, ordinary scaling cannot reach that area.

This is when gum treatment is needed.

It is periodontal treatment that removes tartar and inflamed tissue from deep inside the gums. Put simply, if scaling is surface cleaning, gum treatment is the process of removing the hidden inflammation underneath.

The fact that there are gum conditions that cannot be managed with scaling alone means that the inflammation has progressed deep inside the gums.

Why Scaling Alone May Not Be Enough

Gum disease has stages.

If it is at the early stage of gum inflammation, or gingivitis, it can often be brought back under control with scaling and proper toothbrushing alone.

However, if it is left untreated and progresses to periodontitis, where the inflammation spreads to the jawbone, the situation changes.

The space between the gums and teeth—the gum pocket—becomes deeper, allowing tartar and bacteria to settle inside.

The depth of a healthy gum pocket is usually 3 mm or less. Once it deepens to 4 or 5 mm, neither a toothbrush nor a scaler can reach all the way inside.

That is why gum treatment, which directly addresses the inside of the pocket, is necessary.

How Can I Tell What My Gums Need?

The important point here is that you cannot tell just by looking whether scaling will be enough or whether gum treatment is also needed.

You need to check how deep the gum pockets are and how much jawbone remains.

That is why, when assessing gum health, I measure the pocket depth at each area and also use X-rays to check whether bone loss has occurred.

By measuring the gum pocket depth and examining the condition of the bone through X-rays, it becomes relatively clear whether scaling is sufficient or gum treatment is needed.

This is why, even with the same gum bleeding, one person may only need scaling while another may require gum treatment.

In summary,

scaling and gum treatment are not competing treatments; they simply involve different stages of care depending on the condition of the gums.

It is fortunate when scaling is enough, but if gum problems that cannot be addressed with scaling alone are left untreated, the jawbone may eventually be lost and the teeth may become loose.

Therefore, if you have been advised to receive gum treatment, it likely does not mean that treatment is being increased unnecessarily.
