Gangnam Post-Extraction Implant – Immediate Implant Placement After Extraction Due to Severe Periodontal Disease (Male Patient in His 40s)
Hello.
I am Dr. Ha-seong Kim, director of Yonsei Luminous Dental Clinic.
Today, I would like to introduce the case of a patient who visited us because of discomfort caused by significant looseness and sensitivity in the lower right molar area.

The patient who visited us this time was a man in his 40s.
He had been experiencing discomfort in the area for a long time, but kept putting off treatment because of his busy daily life. As his symptoms worsened, he eventually came to see us. ㅠㅠ

Upon examination, the tooth was very loose, with mobility of grade 2 or higher, and significant inflammation had progressed in the surrounding gum tissue as well.
We first took a CT scan and examined horizontal cross-sectional images viewed from above to determine the location and extent of the inflammation.

In the image, the marked area shows the problematic tooth and the extent of the inflammation.
This area is difficult to identify with the naked eye, but the CT scan showed that the inflammation had spread over a considerable area.

Before placement, we first planned the fixture’s length and position using a CT scan. After placement, we used another CT scan to confirm that the fixture had been inserted correctly according to the plan.
Because severe bone loss had progressed around the tooth root, replacing the tooth with an implant after extraction was a suitable option.
However, rather than proceeding directly with extraction, we carefully assessed whether an immediate post-extraction implant could be placed in the same location.
Post-extraction implant treatment involves performing the extraction and implant placement in a single appointment. It can reduce the healing period and the number of visits, but whether it can be performed depends on the amount of remaining bone and the extent of the inflammation. Therefore, CT analysis must be conducted first.
In this post-extraction implant case in Gangnam, the conditions allowed sufficient bone to be secured after removing the inflammation, so the implant was placed at the same time as the extraction.

The cross-sectional CT scan taken after the post-extraction implant treatment in Gangnam shows that the implant fixture is securely positioned in the area that had been filled with inflammation.
Tooth looseness caused by periodontal disease often has few noticeable symptoms in its early stages, so it is frequently discovered only after the condition has progressed significantly.
If your gums are swollen or bleeding, or if your teeth feel sensitive or loose, we recommend that you do not put off a visit and come in promptly to have your condition checked! ^^
