Fat grafting for sunken upper eyelids: add volume to hollow eyelids!

As we age and the skin undergoes aging, or when excessive dieting leads to a lack of subcutaneous fat,
the volume of the upper eyelids decreases, creating a sunken appearance known as enophthalmos of the eyelids.
It can make you look tired and older,
and it also makes it harder to open the eyes functionally,
which can cause ptosis, so correction is necessary.
In general, sunken upper eyelids are often corrected with fat grafting.
However, because the eyelids move a lot, fat graft survival is low,
so the area may become sunken again without much effect,
and there is a relatively high possibility of adhesion.
To address this issue, at April 31 Plastic Surgery,
we present surgical results with almost no hollowness through orbital fat repositioning surgery rather than fat grafting for eye correction.

Orbital fat repositioning surgery is a method that uses 3D-CT imaging
to pull down the fat hidden behind the eyebrows
and reposition it in the sunken eyelid area.
Even when the eyelids are sunken due to aging or ptosis,
there is still fat hidden behind the eyebrows, and that hidden fat can be pulled down
and evenly repositioned into the sunken eyelids.

General eyelid fat grafting involves harvesting fat from the abdomen, thighs, or buttocks,
then separating it and injecting it into the sunken eyelids.
In such cases, ptosis correction may be needed separately,
and because survival rates are low, more than 50% may sink again within 1 to 2 months.
In some cases, adhesion may also occur.
However, volume-boosting fat repositioning for the eyelids removes the cause of the sunken eyes,
and by making the most of the fat tissue around the orbit and repositioning it,
there is no separate need for ptosis correction and the sunken-eye phenomenon is minimal.
In addition, you can expect a smooth surface without bumps and a semi-permanent effect,
and improvement is possible with one surgery, without additional surgery.

Because the degree of improvement for sunken upper eyelids varies from person to person,
it is important to proceed with a method suited to each individual case.
If the degree of hollowness is not severe, ptosis correction and surgery that advances and repositions the orbital fat alone can restore volume to the upper eyelids and create a brighter eye shape.
If the hollowness is severe, ptosis correction plus orbital fat repositioning + fat grafting,
or fat injection, may be needed. Depending on the condition, micro-fat grafting and dermis-fat grafting can be used to add volume.

For the process of cleanly separating and repositioning fat to go smoothly,
it is advisable to receive treatment from an experienced medical team.
Dr. Oh Won-seok, the eye surgery specialist at April 31 Plastic Surgery,
has accumulated expertise through many clinical experiences and conference presentations on orbital fat repositioning surgery,
so you can expect stable surgical results.
