Hello.
This is April 31 Plastic Surgery.
Many people ask whether removal is possible after autologous dermis surgery.
Today, we’ll talk about autologous dermis removal.
Can autologous dermis placed in the nose
be removed?

Autologous dermis placed in the nose,
can it be removed?
The answer is ‘yes.’
It can be removed well.
Artificial implants such as silicone implants or Gore-Tex do not become established by healing and adhering with the tissue inside the skin. Instead, they heal by forming a thin capsule-like membrane, so removal is very simple.
However, autologous dermis is somewhat more complicated.

Even if the autologous dermis itself has become established,
a plastic surgery specialist with the right technique can separate the dermal layer.
Because separating the flap layer is a technique that specialists perform very often, removal is possible.
The method for removing autologous dermis early and removing it after it has become established is slightly different.
When all of the autologous dermis has become established, partial removal is the right approach.
This is where technique matters.

If it looks too wide, removing the side portions, or if the nasal tip looks puffy or blunt, shaping and removing part of it is done through an open approach.
Without removing it, fine adjustments may also be made.
For example, like the process of refining a carving,
a needle-like instrument may be used to stimulate and break it down so that it is naturally absorbed.
There are also cases where the dermal tissue is broken up through a needle or a thin tube,
or removed while applying a small amount of suction.
Or a method of dissolving it with a well-diluted injection solution may be used.

Some people worry about swelling in the early stage after surgery.
Dermis and fat tissue is absorbed at around 30%, and the timing and degree of absorption vary depending on the patient’s environment, health condition, physical state, stress, and other factors. Keeping this in mind may help you feel less concerned about post-surgery swelling.
If you allow enough time and observe autologous dermis rhinoplasty with the understanding that swelling will last for about 1 to 2 months, you can confirm that the shape settles.
Even after the autologous dermis has become sufficiently established, removal is still possible about 1.5 years later if you want to reduce excessive fullness or puffiness,
but in most cases, rather than removal, partial supplementation or shape improvement is more common.
Why remove autologous dermis
placed in the nose?

Among patients with contracted noses, there are some who undergo reconstruction by placing autologous dermis and then want to make the nose a little higher, but there are almost no cases where the previously placed dermis is removed.
Some people want it removed because of swelling in the early stage, but if you wait until the dermis settles in, you can expect a natural nose shape.

The role of autologous dermis is to
smoothly reinforce thin, uneven skin thickness.
After that, it can later provide a better opportunity by placing an implant or dermis-fat tissue again.

In conclusion, there are no cases where the entire autologous dermis itself is removed,
and I can say that partial removal may be done when only part of it looks blunt or uneven.
