Today, let’s take a look at multiple folds in the double eyelid line,
commonly called a “triple fold.”

A triple fold refers to an unintended crease that forms above the double eyelid line.

There are three major causes of this condition.
① The double eyelid has come undone
② There is a lack of fat and soft tissue above the eye
③ Adhesion has occurred due to surgery
If unwanted creases appear after double eyelid surgery and you are considering revision surgery,
you need to find a solution according to the cause.
In today’s post, we will take a closer look at three surgical approaches based on the cause of a triple fold.

The first cause of a triple fold!
It is when the double eyelid has come undone.
When the double eyelid fixation loosens or comes undone and creates a layered fold, causing a triple fold,
we recommend the Inek Smooth Incisional Method, which adjusts the depth and secures the fold naturally.
The Inek Smooth Incisional Method is also a proprietary technique unique to Inek,
with a trademark application filed with the Korean Intellectual Property Office.
It uses triple fixation at the inner corner of the eye to prevent it from coming undone,
while the middle and outer parts are fixed at a shallower depth so they fold naturally,
and 7 to 9 points are fixed to create a smooth line.

Next, in cases where revision surgery for a triple fold is needed,
the second cause is a lack of fat and soft tissue above the eye.
In this case, “eyelid fat grafting,” which layers micro-fat grafts to prevent clumping, is appropriate.
Our eyelids have skin on the outermost layer,
and beneath that are muscle, other soft tissue, and fat in order.
When there is sufficient volume above the eye, the skin above the double eyelid
covers the lower skin adequately.
However, when fat and soft tissue volume are lacking,
the skin above the double eyelid appears sunken, creating a crease above the existing line.
In such cases, the problem can be solved by increasing volume through fat grafting above the eye.
However, because eyelid skin is thin,
the injected amount must be limited, as the area may appear bulging or feel noticeable to the touch when micro-fat is added.

The final case requiring triple fold revision surgery is adhesion caused by surgery.
If adhesion occurs at the height where the existing line was located after a procedure to lower the double eyelid line,
a triple fold may form.
In this case, releasing the adhered area
and additionally preventing further adhesion with an “adhesion release procedure” is appropriate.
Inek’s adhesion release procedure separates the adhered area,
and to prevent recurrence, fat and muscle are lowered and inserted.
Also, if ptosis is present,
eyelid correction can be performed together to resolve the triple fold.
For reference, in the case of eye revision surgery, it is generally recommended to proceed after 6 months,
when the hard scar tissue has softened.
However, if the symptoms are severe enough to make normal daily life difficult,
the revision surgery may be performed regardless of timing.
