First, you may have heard of phenomena such as multiple-fold eyelids or triple folds,
where the double-eyelid line appears in several layers. What exactly is a multiple-fold eyelid?
A triple fold refers to an unintended crease forming above the double eyelid,
and it describes eyes where multiple lines appear, preventing a smooth eyelid line.
Especially when this multiple-fold eyelid phenomenon occurs after double-eyelid surgery,
it can be worrying and stressful.
What causes multiple-fold eyelids to occur?

There are generally about three main causes.
The first is when the double eyelid becomes undone.
This can happen if the line comes apart because it was not properly fixed after surgery.
The second is when there is insufficient fat or soft tissue above the eyes.
Lastly, a triple fold can occur due to adhesions caused by surgery.
If multiple-fold eyelids occur after double-eyelid surgery or another eye surgery,
revision surgery can be used to address them.
So when is the right time to undergo surgery for multiple-fold eyelids?
Most eye revision surgeries are performed 6 months after the operation.
In general, this is because the hardened scar tissue becomes softer after 6 months,
so surgery is performed after that period.
However, if the symptoms are very severe
and the discomfort is enough to interfere with daily life,
multiple-fold eyelid surgery is performed regardless of timing.

The surgical method differs slightly depending on the cause of the triple fold,
so let’s take a look one by one.
First, if there is not enough fat or soft tissue above the eyes,
the eyelid has skin on the outermost layer, and inside that are muscle,
other soft tissues, and fat in sequence.
When there is sufficient volume above the eyes,
the skin above the double eyelid covers the skin below it,
creating a natural line.
However, if the volume of fat and soft tissue is insufficient,
the skin above the double eyelid can sink inward,
causing wrinkles above the double eyelid and resulting in multiple-fold eyelids.
Therefore, eyelid fat grafting is performed by slowly transplanting microfat
to help prevent clumping.

Second, a common case is when the double eyelid has come undone.
When the fixation of the double eyelid loosens or comes apart,
re-fixing the double eyelid can resolve the problem.
