Hello, this is Inique.
Today, we’re going to learn about
double crease eyelids.
A double eyelid refers to the fold above the eye.
When there is only one fold,
it is called a double eyelid, but when an additional fold
unwantedly forms above it,
this is called a double crease eyelid.
Double crease eyelids can make the eyes
look heavy and tired,
and this can even lead to a psychological
decline in confidence.
Sometimes they can also obstruct
vision,
and they may cause various inconveniences
from both aesthetic and functional perspectives.

There are several causes of
this double crease eyelid, but the biggest reasons are
when the double eyelid fold loosens,
when there is insufficient fat above the eye,
and adhesions caused by surgery.
There can also be various other causes.
Many people wonder,
"Why does a double eyelid fold loosen?"
If you have thicker skin,
the chances of it loosening are even higher.
The reason is that a double eyelid fold,
as mentioned earlier,
is a crease formed when the skin folds,
which is called a double eyelid.
If you have thinner skin,
like folding thin paper, there is not much resistance,
so the double eyelid forms easily
and does not loosen easily.
But
if you have thicker skin,
like folding thick paper, the resistance is stronger,
so the double eyelid tends to loosen more easily.
Or, when the strength to open the eyes is weak,
the double eyelid can also loosen easily.

In such cases, if eyelid correction is used to help the eyes open properly,
the possibility of the double eyelid loosening can be reduced.
That is why,
when a double crease eyelid forms because the double eyelid fold has loosened,
incision surgery is recommended if the original procedure was a buried suture method,
and
for those whose folds have loosened even after incision surgery,
the fixation is performed more firmly.

If there is insufficient volume in the soft tissue above the eye,
it is important to restore the volume.
There are two ways to do this.
One is the fat repositioning method, which spreads your own fat
and moves it downward.
Our eyes naturally have fat.
In some people, the amount is small
and it sits higher up.
In such cases, the fat above is dissected,
drawn downward, and fixed to the upper tarsal plate,
so that the fat volume is sufficiently moved downward
to cover the area.
This is the fat repositioning method.
The other method is fat grafting, which involves harvesting fat from the abdomen or another area
and transplanting it to the eye area because fat alone in the eye may not be enough.
Fat grafting means taking fat from another area
and filling the volume above the eyes.
If fat grafting is performed,
it can help resolve hollowness
and improve double crease eyelids.
Many patients ask whether fat grafting is really necessary.
If the amount of fat you naturally have is very small,
even if fat repositioning is performed,
the amount of fat may still be insufficient,
which can lead to lower satisfaction after the procedure.
Therefore, for those with severe hollowness around the eyes,
fat grafting is recommended more often.

Revision surgery for double crease eyelids is generally recommended
from six months after surgery, when the hard scar tissue has softened,
but if the symptoms are severe and daily life is difficult,
revision surgery may also be performed
after consultation with the medical team.
