If too much lacrimal caruncle is exposed
and the correction is excessive,
it can create an unnaturally sharp impression,
so this must be taken into account during surgery.

At present, there are two main methods
for epicanthoplasty revision for reconstruction
at a place known for revision eye surgery.
The first is epicanthoplasty reconstruction.
The second is mini epicanthoplasty revision surgery.
The epicanthoplasty reconstruction method
restores cases where the amount of exposed lacrimal caruncle is excessive,
and it can naturally reshape the front of the eye.
Mini epicanthoplasty can be performed
when epicanthoplasty has already been done,
but the inner corner of the eye is still round
and gives a cramped impression.

The purpose of epicanthoplasty reconstruction at Inique,

is to rebuild the exposure of the lacrimal caruncle
that was created too aggressively.
It can be performed to improve cases of overcorrection
beyond the ideal exposure amount,
where more than 2/3 has been exposed.
The reconstruction method is performed using a V-Y advancement technique

An incision is made according to the scar,
and after dissection to allow movement,
it is sutured closed in a “zipper-like” pattern,
with about 1/3 closure considered ideal.

There are cases where epicanthoplasty was performed,
and even though the amount of lacrimal caruncle exposure is not excessive
and is appropriate, the eyes still look cramped.
In such cases, a more delicate examination is needed.
When performing epicanthoplasty,
not only must the amount of lacrimal caruncle exposure be considered,
but the shape of the inner corner of the eye
must also be taken into account.
So even when the amount exposed is appropriate,
the reason it still looks cramped
is because the inner corner structure of the eye is round.

In such cases, mini epicanthoplasty revision surgery
can reconstruct the area without excessive removal,
only lightly refining the Mongolian fold.
If done this way,
it is possible to preserve the existing amount of lacrimal caruncle exposure
while still creating a more open and uncluttered look.
