Hello, this is Diu Plastic Surgery.
When you look in the mirror, the first thing that catches your eye is, more often than not, the jawline.
The cheeks may have slimmed down, but the stubborn fat that remains under the chin,
or the embarrassment of seeing a double chin clearly in the side profile even though the front view looks fine in photos
But this so-called jaw fat is not simply a matter of weight or age.
Because multiple factors act together, including the structure of the facial bones, the distribution of fat, skin elasticity, and lifestyle habits,
the concern of “I lost weight, so why is the jaw fat still there?” often becomes a common question for many people.
In cases like this, liposuction can be a tool that does not simply remove fat, but helps create a smoother, more defined balance in the facial contours.
In particular, the fat under the chin is not a single layer but has a dual structure above and below the platysma muscle, so removing only the superficial fat may not make the overall line look clearly defined.
Also, depending on the case, a small amount of autologous fat may need to be grafted into sunken areas
to complete a more three-dimensional and harmonious contour, which is what creates the impression of a truly “small face.”
Therefore, in this post, I will explain how liposuction should be performed for effective results by classifying specific cases.

‘Jaw Fat Liposuction’
Is Not the Same for Everyone

Even though it is a procedure to reduce jaw fat, not everyone undergoes it in the same way.
Depending on facial structure, skin elasticity, the depth and amount of fat, and age group, the suction area and depth, the equipment used, and any combined procedures will naturally differ.
It can be broadly divided into three types.
Those in their 20s to 30s whose facial contours are unclear

Overall, I’m not really fat,
but my jawline always looks blunt
The cheeks have slimmed down,
but the line under the chin is not clearly defined
For people like this, it is often not that there is a large amount of actual fat, but rather that the balance of volume distribution and skin elasticity has become disrupted, making the facial line appear unclear.
In such cases,
- Liposuction is performed on areas with excessive volume,
- Sunken areas (nasolabial folds and marionette lines) are filled with fat grafting
and it is important to redesign the contour by redistributing the fat across the entire face.
Along with this, a mini lift using a temporal incision (the ‘Cat Lift’) can be combined to expect a more definite result than thread lifting or skin tightening.
Cases concerned about skin sagging after contour surgery

I had contour surgery and my bones look slimmer,
but the remaining skin actually makes me look older
Recently, among people in their 20s and 30s, patients who feel dissatisfied due to skin sagging after contour surgery have been steadily increasing.
In this case, simple liposuction alone cannot resolve the sagging, so a lifting procedure using the deep plane facelift method is performed in combination.
This procedure lifts the facial tissues from a deeper layer than the SMAS, improving cheek sagging and restoring a sharper jawline.
At this stage as well, if CT shows a large amount of buccal fat, removal of the buccal fat through an intraoral incision can also be performed together, allowing a clear V-line to be created even without contour surgery.
Age-related jaw fat in middle-aged and older adults

As I get older,
it feels like my whole face is collapsing
The jaw fat is a concern,
but even my forehead and eye area have sagged
In this case, rather than simple liposuction, the basic approach is precise lifting combined with deep plane facelift and extended SMAS facelift.
Fat in the jaw area is suctioned and removed, while autologous fat from the liposuction is grafted into sunken nasolabial folds and the eye area.
By redistributing the facial volume and flattening the contours, both lifting and volume enhancement are achieved at the same time.
If a lower blepharoplasty or brow lift is performed together, it is possible to achieve not just jaw fat removal but also a total anti-aging effect.
At this time, the incision scars are hidden along the natural facial wrinkle lines, and scarring is minimized with delicate suturing for a faster recovery.

Jaw fat is not simply a matter of whether it looks good or not.
It is also the final puzzle piece that determines the overall impression of the face.
How much volume to reduce, where to leave it, and how to redistribute it in a three-dimensional way—these small differences create the sharpness of the jawline and the clarity of the impression.
This is also why it is necessary to establish a specific procedure plan through a clear direction rather than simple liposuction.
And that precision is what ultimately makes a person’s face look the most natural and beautiful.
That is all the content I prepared. Thank you for reading this long post.
