What were the actual patient’s concerns?

First, the patient who underwent surgery was a 25-year-old woman who had been concerned about a hump nose and a low nasal tip. She wanted to improve her hump nose and long nose, and raise the tip of her nose so it would look natural.

How is the design for hump nose and long nose surgery carried out?

For 3D fitting nose surgery for a hump nose and long nose, the patient’s condition is first checked before surgery, and sufficient consultation and analysis are conducted.

This is because, even with the same surgical method, the surgery is performed with a 3D fitting design tailored to each individual.

The patient had a nose with a low nasal tip and glabella area, as explained earlier. The consultation was conducted with a plan to remove the hump first, slightly raise the nasal tip, and then build the bridge accordingly.

However, if surgery is performed this way, the nose can become somewhat longer. If the nose becomes longer, it can make the face look larger. After rhinoplasty, the face should look smaller, so it would not be desirable if it looked larger. Therefore, the plan had to be revised to prevent the nose from becoming long after surgery. To avoid a long nose, the most important point is where to set the starting point of the nose. Taking the patient’s facial characteristics into account, the starting point of the nose was designed to begin slightly above the pupil of the black eye. In addition, considering that the patient’s nasal tip was blunt, it was decided to proceed by slightly elevating the tip point as well so that the nose would not appear long after surgery.

How is the implant chosen for hump nose and long nose surgery?

After the design is decided, the next step is determining which implant to use. If a standard implant is used, the nose may be shaped to fit the implant rather than the implant being tailored to the individual, resulting in a uniform design. Also, if the implant length is not a perfect match for the patient, there is a possibility that the nose may become long after surgery. Therefore, 3D fitting is performed for customized surgery, and the thickness and design of the implant are selected to match the patient’s facial proportions. For this patient, a 3 mm implant was used. In particular, in addition to proportional adjustments such as length and raising the tip, osteotomy was also performed to narrow the nasal bones.

What did it look like 1 week after surgery?

This is the appearance one week after surgery. At one week after surgery, the stitches are removed, and since not much time has passed since the operation, there is still quite a bit of swelling.

From the side, you can also see that the starting point of the nose is quite swollen. However, this level of swelling is not excessive for the first week. Also, the nasal tip may appear lifted so that the nostrils are visible, but this does not mean the surgery actually caused that; rather, it looks lifted because the philtrum is swollen, so it is not a major issue.

In the front-view photo, the area inside the red circle is the bone osteotomy site, and even though it is swollen, the shape is well maintained. The area marked with the white circle is relatively soft tissue, so it appears more swollen compared to the harder osteotomy area. If the nasal tip already looks slim at one week, it can become sharp like a blade after about six months. Therefore, if the swelling is at the level shown in the photo, you can consider it an appropriate result.

What does it look like one month after surgery?

At one month, the glabella area becomes the most prominent when viewed from the side. The glabella becomes the most protruded at around one month. Initially, the glabella is swollen due to swelling, but after about a month, a hard surgical scar forms under the skin. This surgical scar is most pronounced at one month, which is why the glabella looks swollen.

However, by six months, the swelling will ease, the nasal tip will come down a little more, and the glabella swelling will also subside significantly. At six months, the starting point of the nose will be seen as the starting point of the implant, and the overall length of the nose will be shorter than at one month, becoming more proportionate.
