
Worried About Swelling After Nasal Osteotomy? Apgujeong Plastic Surgery Ultrasound Osteotomy! Interview (Part 2)

One method is the open osteotomy introduced in the previous post,
which is not simply a surgery that cuts the bone, but a method that lowers the recurrence rate by
visually confirming the osteotomy area and completely cutting it at the exact location.

the “ultrasonic osteotomy.”





Q. Why is Globi’s osteotomy special?
Director Kim Jae-in’s ultrasonic osteotomy may sound somewhat unfamiliar.
Ultrasonic osteotomy is a surgical method that cuts bone using an ultrasonic saw.
The word “saw” may sound a little frightening, but it is not the metal saw people usually imagine.
Because it cuts the bone using an ultrasonic saw emitted from the tip of a device,
it protects the mucosa and surrounding tissue around the osteotomy site from damage while cutting the bone,
so it can be considered a safe osteotomy method.



Traditional osteotomy using a chisel can damage the mucosa and surrounding tissues,
leading to significant bleeding and possibly severe swelling after surgery.
However, ultrasonic osteotomy, like open osteotomy, directly confirms the osteotomy site visually
and precisely cuts only the bone without damaging the surrounding tissue or mucosa,
so it can be said to be the most ideal method in that postoperative swelling may be less.
Chief Director Noh Bong-il also mentioned that osteotomy is performed using a chisel, so you may wonder,
“Isn’t ultrasound better?”
At Apgujeong Plastic Surgery, Chief Director Noh Bong-il uses a narrow chisel with a width of about 1 mm,
so unlike conventional osteotomy, the risk of mucosal and tissue damage is relatively low.










Q. Who is the most memorable patient?
Chief Director Noh Bong-il answered, “A patient whom I performed osteotomy on.”
He was referring to a patient who had already undergone osteotomy and expressed concern that he wanted the nose to become wider again.
It was a case that gave him the opportunity to directly check whether the bone from the previous osteotomy had properly fused,
and also to confirm whether already cut and fused bone could be returned to its original state.
He said that after osteotomy, the bone was firmly and securely fixed by fibrous tissue called “fibrous connective tissue,”
which connects structures in the body.
However, bone that has been cut cleanly, remains well maintained, and has fused well can be a poor condition for restoring it to its original state.
In Chief Director Noh Bong-il’s case, fortunately, he was able to re-tap the bone and fix it by pushing it up from the inside,
safely meeting the patient’s needs.
