Unlike implants, fat grafting uses fat from your own body, so natural results can be expected.

At the same time, fat retention rate is always a variable.

And that retention rate is a key factor that determines the outcome of the surgery.

In fact, even with the same fat grafting procedure, results can vary quite a bit depending on injection technique and the fat purification process.

In other words, the details of the process, more than the name of the surgery, are what determine the result.

In this post, we would like to organize some points that may be helpful for those preparing for breast fat grafting.

Fat grafting: the difference in results starts with 'how it is injected.'

When discussing retention rates in fat grafting, there is a topic that cannot be left out.

It is the injection method.

Doctors each have slightly different opinions on this point. I, too, have thought about various methods for a long time.

Through that process, one thing became clear to me.

If a large amount of fat is injected into one area all at once, a poor environment for retention can be created.

Let me explain why.

Transplanted fat cells can survive only if they receive oxygen and nutrients from the surrounding blood vessels.

But if the fat is concentrated in one spot, it becomes difficult for blood vessels to reach the center.

The cells on the outer side may survive, but the cells in the inner area do not receive enough nutrients and eventually die.

As a result, that area gets absorbed, leading to a decrease in volume.

If you clearly put in a sufficient amount but still feel that it shrank, there is a good chance that this kind of mechanism was at work.

That is why we use a micro cannula to inject small amounts distributed across multiple layers.

This method is called the triple retention method.

With this approach, each fat particle has a larger area in contact with the surrounding tissue, and there is naturally more room for new blood vessels to grow in.

In that way, a more favorable environment for retention is created.

Just as seeds sprout well when they are evenly scattered in fertile soil, fat also survives better when distributed at the right density.

To be honest, this method takes more time.

Even when injecting the same amount, it must be done much more carefully than a single-shot injection, and because the surgery takes longer, the level of difficulty for the surgeon also increases.

Even so, we find it difficult to choose a path that cuts corners in this process.

If this difference in precision leads to the surgical result, then no matter how much effort it takes, we believe it is right to follow the principles.

A way to further improve retention: SVF

Earlier, I explained how important the injection method is for retention. In fact, we apply one more thing here.

It is SVF, or Stromal Vascular Fraction.

Many of you may be hearing this for the first time, so let me explain it a little more simply.

SVF is a component that originally exists within adipose tissue.

After collecting the fat, it can be separated through enzyme treatment and centrifugation. It contains various cellular components involved in blood vessel regeneration and tissue recovery.

Why is this important? As mentioned earlier, for transplanted fat cells to survive, new blood vessels need to connect quickly.

But under natural conditions, this process of blood vessel formation is slower than you might expect.

If oxygen supply is cut off during that gap, the cells may not be able to hold out.

However, if SVF is mixed with purified fat and injected together, it may help promote this blood vessel formation process.

Simply put, it helps create the environment first so that the transplanted fat can take root quickly in its new place.

You can think of it as preparing the space where the fat will go in advance.

In fact, extracting SVF requires a far more difficult and delicate process than ordinary grafting.

The extraction takes longer and requires more work, but the reason we, as medical staff, adhere to this principle is clear.

If we can create even a slightly better environment for retention, we believe the extra effort is worth it.

It is not an easy choice for me to skip a process that may affect the outcome simply for convenience.

Of course, applying SVF does not guarantee results 100%.

Still, if you are considering breast fat grafting, be sure to ask during your consultation whether SVF is used together with it.

That alone can be an excellent criterion for choosing a clinic.

I wrote this in the hope that it would be of some help to those considering breast fat grafting.

With any surgery, the factors that affect the result are often found in the details,

and how well those details are followed is often what determines satisfaction in the end.
