When people come in for consultations because they are concerned about breast volume,
they often say this:
“I’m uneasy about implants,
so can’t I just do fat grafting?”
That is not wrong,
but this question is missing one very important premise.
That is,
you should first determine whether
“fat grafting is the right method for me.”
Implants and fat grafting are
not simply different materials.
The surgical methods are different, and not only the nature of the results,
but also the suitable candidates are different.
So if individual body type conditions are not taken into account,
satisfaction may be low,
which is why a careful decision is needed.
In this post, I will directly compare the two methods,
and honestly explain who breast fat grafting suits well
and who it does not.

and people for whom it is not
Breast fat grafting
When I consult with patients,
there are moments when I have to say this:
I understand that you want fat grafting,
but honestly, with your current body type,
it may be difficult to achieve the result you want.
Of course, it is not easy to say that.
But if there is a gap between the direction you want
and the results that are actually possible,
I think it is right to tell you in advance.

So when we consult,
our clinic tends to check the limitations of fat grafting first,
rather than its advantages.
The most important condition is whether there is enough fat that can be harvested.
Generally, at least 150–200 cc per breast needs to be injected
to noticeably feel a change in volume,
and to secure that amount,
the area from which fat will be collected—
that is, the abdomen or thighs—must have a sufficient fat layer available for extraction.
If you have a slim body type or a low body fat percentage,
harvesting may be difficult,
and if the amount is increased too much,
there can be problems such as unevenness at the liposuction site.

In practice, there are also people who wanted fat grafting
but changed direction to implants after consultation
because their body type conditions did not fit.
They are disappointed at first as well,
but as time passes, many of them say that the choice
was the right direction for them.
On the other hand, there are clearly cases where fat grafting is a good fit.
If you have enough fat,
and want natural volume rather than a dramatic size change,
fat grafting can be a good choice.
The same is true for people who strongly dislike foreign materials.
In the end, it is not a matter of whether fat grafting is good or bad,
but of whether it is the right method for your current condition.

Let me compare the two methods in a bit more detail.

The biggest strength of implants is that
they can create the desired volume predictably.
You can set the size with a fair degree of clarity,
and there is less worry that the volume will significantly decrease after surgery.
However, because this method involves placing a foreign object into the body,
there are aspects that require long-term management,
such as capsular contracture or rupture.
Depending on the placement site or incision method,
the recovery period also varies.
Many people who find these aspects burdensome
become interested in fat grafting,
and I fully understand that feeling.

Fat grafting has the advantage of causing less psychological burden,
because there is no foreign material,
and it can provide a more natural volume.
But there is one point I always make sure to mention in consultations.
Some of the transplanted fat is absorbed by the body over time.
This does not mean the surgery was done incorrectly;
it is a natural characteristic of fat grafting.
So the result a few months after surgery,
rather than immediately after surgery,
is closer to the final outcome.
If you know this in advance,
you will not be caught off guard after surgery.

Many people come in without knowing this,
so I always explain it in advance during consultations.
In consultations, I do not say that one method is better than the other.
That is because some people are a good fit for implants,
and others are a good fit for fat grafting.
What matters is not deciding on the surgical method first,
but matching the result you want now with your body type conditions.
So if you tell me what kind of result you want,
we can find the right direction together.
