As you look into nose revision surgery,

the direction of your search terms changes at some point.

At first, you search for a place that does nose revision surgery well,

but then you end up searching for nose revision surgery side effects.

After experiencing one failure, it is natural for the second surgery to

feel much more frightening.

Unfortunately, though, there are not many places that properly explain where

and why side effects occur.

So today, I will cover two things: why side effects happen in nose revision surgery,

and what you should check before surgery

to avoid them.

If you read to the end, at least

you will be able to make your decision

knowing where problems can arise.

Before talking about side effects from nose revision surgery,

there is one thing I would like to point out first.

Revision surgery itself is not what is dangerous;

the biggest variable is proceeding with surgery without accurately understanding

the cause of the side effects.

You need to know the cause in order to prevent the same problem from recurring.

The most common causes of side effects in nose revision surgery are

broadly divided into two categories.

Problems with the material, and problems with the surgical method.

These two are more closely connected than you might think.

First, from the perspective of materials,

the L-shaped silicone that was commonly used in the past

is a representative cause.

An L-shaped implant is a structure that runs continuously from the bridge of the nose

to the tip.

The tip of the nose is a part that moves every time you make facial expressions.

If a hard implant extends all the way to a location with so much movement,

it will continuously press on the cartilage at the tip of the nose over time.

As a result, the cartilage may become deformed,

or the skin at the tip of the nose may become thinner,

causing the implant underneath to become visible.

In severe cases, this can lead to contracture.

Contracture is a phenomenon in which tissue shrinks as an excessive fibrotic response occurs

at the surgical site, causing the nose to be pulled upward.

Once this happens,

simply replacing the implant will not solve the problem.

The thick scar tissue must first be carefully dissected,

and adhesions must be released.

This is why revision surgery becomes

much more difficult than the first surgery.

There are also issues with the surgical method.

If the bridge and the tip of the nose are addressed at the same time

with a single implant,

the silicone will come into direct contact with the tip of the nose without any autologous cartilage.

The longer a foreign material, not your own tissue, stimulates the highly mobile tip of the nose,

the higher the risk of side effects.

For this reason, the current commonly recommended approach is a separated design: using an implant for the bridge,

while the tip of the nose must be finished only with autologous cartilage.

The reason I am telling you this

is not to tell you that you must come to our clinic.

If you are considering revision surgery,

I recommend that you first check, at any clinic,

what materials were used in the previous surgery and how they were used.

What to check to lower the likelihood of side effects

So what should you look at next?

If silicone was in direct contact with the tip of the nose,

a simple implant replacement will not be enough.

You first need to check the condition of the cartilage at the tip of the nose,

which has been under pressure for a long time, and if it has been deformed,

that must be corrected before reconstruction.

Also, if contracture is present, unless the hardened capsular tissue is sufficiently removed,

the same problem can recur even if a new implant is inserted.

This capsular removal process is called capsulectomy,

and how cleanly it is performed

has a major impact on the revision surgery outcome.

This is exactly why revision surgery takes longer

and is more complicated than the first surgery.

The second thing to check is

how much autologous cartilage remains.

Autologous cartilage is needed to rebuild the tip of the nose in revision surgery,

but if septal cartilage was already used in the previous surgery,

ear cartilage or rib cartilage must be used instead.

In consultations, I review the previous surgical records together

to identify what materials are still available.

The direction of nose tip reconstruction changes depending on

which cartilage can be used.

Therefore, if you have the consent form or records from your previous surgery,

bringing them to the consultation will allow for much more specific discussion.

One reason revision surgery feels frightening is that it has more variables

than the first surgery.

That is because the internal tissue

has already been altered once.

But identifying those variables in advance

and reflecting them in the surgical plan,

I think that is the difference between whether or not a doctor has revision surgery experience.

It is natural to be afraid of nose revision surgery.

You already have the experience of expectations being broken once.

But once you know why side effects occur

and what to check next,

that vague anxiety can turn into confidence.
