One of the questions we hear most often in the consultation room is
this one.
It is said to be scar-free,
so will there really be no trace left?
But in practice, there are not many places that clearly explain
how this is possible,
or whether it really leaves no scar.
In fact, the issue of scarring should begin with skin characteristics
rather than the surgical method.
Once you understand why East Asian skin is more prone to scarring,
and how surgical planning changes to account for that,
you will be able to judge much more accurately
what the term “scar-free” actually means.
Why Scars Form More Easily in East Asian Skin

When considering philtrum reduction surgery,
it is natural to worry about scars first.
But there is a scientific reason this concern is not just a vague fear.
Most plastic surgery textbooks and papers
are written based on Western patients.
For example, in white patients, scars after skin injury
tend to blend more naturally with the surrounding skin tone,
so in the West, a direct incision under the nose
has long been used as a standard approach.
It was effective, and the scar burden was not very high.
However, East Asian skin is structurally different.
Because it contains relatively more melanin,
it is more prone to post-inflammatory hyperpigmentation after skin injury,
and the skin tissue often regenerates excessively,
so scars are much more likely to stand out.

This is the key factor that causes differences in outcomes
even when the same surgical method is used.
As those experiences accumulated,
a separate surgical approach for East Asian patients
began to develop.
So now, the goal is not to eliminate the incision itself,
but to move the incision to a less noticeable location.
If the incision is placed inside the nostril,
it is often difficult to see any trace from the front.
If you understand that the expression “scar-free”
does not mean “absolutely no scar,”
but is closer to the idea of “hiding the scar as much as possible,”
you will have a much more accurate standard
when comparing surgical methods.
Why Hiding the Incision Is Not the End

Why not just make the incision inside the nostril?
This is a question we hear often in consultations.
Even if the incision method is chosen to hide scars,
that alone does not complete a scar-free procedure.
The incision location is only the starting point,
and to reduce the burden of scarring while maintaining stable results,
internal design must support it as well.

If philtrum reduction surgery only removes skin,
there is a risk that the philtrum will lengthen again over time.
That is because the skin has elasticity that makes it
want to return to its original position.
Therefore, at the same time as removing the outer skin,
it is very important to combine this with a method that firmly secures and lifts the philtrum tissue from the inside using sutures.
Only when internal tissue fixation is added can the downward pull of the skin be controlled.
The combination of pulling from the outside and securing from the inside
is what makes the results more likely to remain stable.
This internal fixation work requires a great deal of precision.
If the position and tension of the suturing differ even slightly,
the philtrum may appear asymmetric
or leave a sensation of tightness.
That is why, during the preoperative consultation,
it is a good idea to check not only the incision location
but also how the internal fixation will be designed.
No matter how much surgical methods advance,
the possibility of side effects always exists,
and reducing that possibility
ultimately begins at the preoperative planning stage.

So during consultations,
we also clearly say, without hesitation,
“for these patients, we do not recommend surgery.”
For example, this includes people whose nostrils are already significantly upturned,
such as those with a “snub nose” shape,
or those who have very little extra skin in the philtrum area.
In such cases, if you focus too much
on shortening it aggressively,
issues such as nasal shape distortion or functional discomfort
may become a bigger problem than hiding the scar.
What matters more than making it “shorter at all costs”
is that the overall balance of the face is not disrupted
after surgery.

Before checking whether a design that simply hides scars is possible,
first make sure that a surgical plan is being established
that matches your skin characteristics and structure.
