After receiving a consultation for under-eye fat repositioning,
some people say this.
At other places,
I was told that only the fat needs to be moved,
so why do you say the tear trough also needs to be treated here?
That is a very reasonable question.
You may understand the idea of repositioning the fat,
but when you hear about tear trough release as an additional step,
you may worry that the surgery is becoming larger than necessary.
But once you understand what happens if this step is left out,
and in which cases it is truly necessary,
you will likely find it more convincing.
Why moving only the fat is not enough

This question is actually very natural.
Just hearing the term under-eye fat repositioning
can make it sound as if moving the fat is all there is to it.

But in many cases, the outcome of the surgery
depends on the stage before repositioning, which is release.
Under the eyes, there is a structure called the tear trough ligament.
This ligament serves to firmly attach the skin to the bone.
It helps maintain the shape under normal conditions,
but as time passes, the surrounding tissue sags downward while
this ligament remains in place, causing the skin at that point to look sunken.
It is similar to fixing the center of a canopy with a nail,
which makes only that part appear indented.
The problem is that if the ligament is left as it is and only the fat is moved,
the bulging area may decrease, but the sunken groove remains.
Since the ligament continues to pull the skin downward,
even if fat fills the area above it, the boundary line does not disappear.

That is why tear trough release,
in other words, the process of loosening this ligament,
needs to come first.
Only when the ligament is released can the repositioned fat
fill the sunken area evenly,
allowing the entire under-eye contour to flow smoothly.
However, not everyone needs release.
If the tear trough depression is not significant compared with the degree of under-eye fat bulging,
fat repositioning alone may be sufficient.
On the other hand, if skin laxity is also present,

Considering a realistic recovery period,

There are cases where people say, “I’ll rest for just two days and return,”
only to contact us again on the third day.
This is because swelling can increase more than expected,
and since this varies from person to person, it is much more comfortable to plan with extra time from the start.

Something that is more important than you might think during recovery is your sleeping position.
In the early stage, it is better to keep your head slightly elevated with a pillow,
higher than your heart.
This helps reduce blood flow toward the face,
which makes a real difference in managing swelling.
If you lie on your side and only one eye area gets pressed,
a difference in swelling between the left and right sides can occur,
so we recommend sleeping on your back if possible.
Even if it is uncomfortable, paying enough attention to these details
can make the recovery speed noticeably different later on.
Since under-eye fat repositioning can create a clear change in appearance,
it is important to decide only after understanding the procedure accurately.
