Does revision surgery mean you have to fix absolutely everything from the start?

This is a question that comes up very often in consultations. Every time we hear it, we can clearly feel the countless worries you went through before deciding on revision surgery, and the fear of “What if it fails again this time?”

Because we understand that anxiety well, we want to make this part as clear as possible.

To put it simply, revision surgery does not mean that everything must be touched again.

The more unnecessary work is done, the more tissue damage accumulates, and the fewer options remain later on.

At Globi, we make it our principle to identify only the problematic areas accurately and correct them within the smallest necessary scope.

If the bridge line is fine but only the tip has sagged, we leave the bridge implant in place and rebuild only the tip tissue.

Conversely, if the implant is distorted, we may solve it by replacing only the implant without touching the tip.

This means the scope of correction does not automatically become wider just because the result is unsatisfactory; instead, it is determined after carefully checking the internal condition during surgery.

That said, there is something we should be honest about.

If the skin is too thin and the cartilage is visible, or if contracture is severe and internal adhesions are extensive, simple correction becomes difficult and complex reconstruction is unavoidable.

In such cases, even if we want to reduce the scope, it is not possible to do so, which is why a specialist’s diagnosis comes first.

It usually takes about 6 months for the internal nasal tissue to stabilize and for scar tissue to soften.

The idea that rib cartilage is always used in revision surgery is widespread, but in reality, the first step is to check the condition of the tissue remaining inside the nose.

If only one side of ear cartilage was used in the first surgery, the other side may still remain,

and if only ear cartilage was used, the septal cartilage may still be intact.

If, after separating and checking, the damage is not severe, there are cases where repositioning alone is sufficient without additional harvesting.

Rib cartilage is the last option, used when structural support is truly insufficient.

If rib cartilage is needed, the next question is whether to choose autologous or donor cartilage.

The autologous method uses cartilage harvested from your own rib.

Because it is your own tissue, the risk of rejection is very low, and the absorption rate is low, so it has the advantage of maintaining a firm shape for a long time.

Of course, you may be concerned about the chest incision. For women, a minimal incision is made along the inframammary fold, so although there is individual variation, in most cases it becomes inconspicuous over time.

The donor method has the advantage of not requiring harvesting, but the absorption rate is relatively higher, and in rare cases, reports of the nose tip lowering or inflammation have been noted.

In terms of long-term durability, it is true that the autologous method is more advantageous.

Also, when rib cartilage is used as a structural material for the nose tip or bridge, if it is inserted as a long single piece, a phenomenon called “warping” can occur. Because cartilage tends to return to its original shape, the nose may appear bent over time.

To prevent this, a method of cutting it into small pieces is also used, but this has the limitation of making it difficult to achieve a smooth line.
