after deciding to undergo revision surgery,
quite a few are actually cases
where it is still okay to wait.
When someone visits around five months after surgery,
saying, "I think my nose has become too low,"
we often find, after checking their progress,
that the swelling has not completely subsided.

It takes at least three months, and up to six months,
for the final results of rhinoplasty to settle.
Changes in the height or shape felt during this period
are greatly affected by swelling,
so rushing into revision surgery at this time
means intervening again before the internal tissues
have had enough time to recover.
Because revision surgery is more difficult than the initial procedure,
proceeding when the body is ready
is much more favorable for the result.

That does not mean that waiting unconditionally is always the answer.
There are signs that require more prompt attention.
If the skin at the tip of the nose becomes thinner
and the cartilage inside begins to show through,
tissue damage can become more extensive the longer you wait.
If you feel that your nose is being pulled upward—
in other words, if you notice symptoms of contracture—
this may be a sign that scar tissue is contracting internally,
so early evaluation is important.
If the implant tilts to one side
or the shape of the nose suddenly changes,
its condition should also be checked promptly.

The choice of material in revision surgery

and what is needed?"
The first thing we check during a consultation
is the condition of the existing cartilage.
When the cartilage used previously is separated
during the actual surgery,
if its shape is intact,
there are cases where the desired structure
can be created simply by repositioning the existing material
without harvesting new cartilage.
If cartilage was taken from only one ear during the first surgery,
we may use the opposite ear.
If only ear cartilage was used,
we may newly use the septal cartilage inside the nose.
Only after reviewing both of these options
and determining that additional support is still insufficient
do we choose rib cartilage.

Another point that must be addressed along with the material
is the surgical design.
Separating the bridge of the nose and the tip
when performing surgery is currently the standard approach for revision,
and the reason is simple.
The tip of the nose is an area that moves whenever you make an expression.
If a hard implant extends into the tip,
it cannot move along with it,
and over time it can press on the cartilage at the tip
and alter its shape.
Therefore, while the bridge is supported with an implant,
we always finish the tip with biocompatible autologous cartilage.
However, there are also points we need to explain honestly.
If the skin at the tip of the nose has already become very thin,
or if internal tissue damage is extensive due to repeated surgeries,
it may be difficult to achieve the desired result.
In such cases, rather than recommending surgery,
we believe it is right to explain the condition thoroughly first.
The longer you spend considering revision surgery,
the heavier the emotional burden naturally becomes.
