Won't your eyes look bigger if you only create a double-eyelid crease?
Many people come in thinking this way.
However, when we actually conduct a consultation,
the cause of a sleepy appearance is often not
the absence of a double-eyelid crease—far more often than people expect.
If surgery is performed after identifying the wrong cause,
it can be difficult to be satisfied with the results,
and in many cases, it can lead to revision surgery.
Before deciding to undergo double-eyelid surgery,
we will go over the things you should check first.

“I have a double-eyelid crease,
so why do my eyes still look so small?”
At first, this question may sound simple,
but with years of clinical experience, we often find
that a fairly complex situation is contained in these few words.


One of the most common causes of a sleepy-looking appearance
is ptosis.
When the muscle that lifts the upper eyelid,
the levator palpebrae superioris, becomes weak, the upper eyelid excessively covers and presses down over the pupil,
making the eyes look small and heavy.
Simply put, this is a condition in which the force used to open the eyes is insufficient.
People with this condition unconsciously raise their forehead muscles
in an effort to open their eyes, so they tend to have particularly deep forehead wrinkles
or eyebrows that sit higher than usual.
In fact, some people who visit us for Botox consultations because of forehead wrinkles
are unexpectedly diagnosed with ptosis.
What happens if someone with ptosis
only gets a double-eyelid crease?
A crease is created, but the eyelid still hangs down,
which can have the opposite effect and make the double-eyelid crease look even more constricting.

Therefore, in such cases, the double-eyelid surgery should be planned together with ptosis correction,
which strengthens the muscles used to open the eyes,
in order to achieve a more open and defined appearance.
Although the recovery period varies depending on the surgical method and the individual's constitution,
in many recent cases, even when ptosis correction is performed through a small-incision or non-incisional method,
most of the major swelling subsides in about a week,
allowing a smooth return to daily activities.
When we explain this, many people finally feel reassured.
Rather than regretting the decision later, we believe it is better to address the cause from the beginning,
so we generally do not recommend procedures that are not appropriate.

“How can you tell during a consultation?”
If you know in advance what process is used to identify the cause during a consultation,
you can communicate with us more comfortably.


The first thing we examine is how much the upper eyelid covers the pupil
when the eyes are open.
When looking straight ahead, the upper eyelid normally covers
approximately 1–2 mm of the upper part of the pupil.
If it covers more than this,
we consider it a sign that the force used to open the eyes may be insufficient.
At the same time, we also examine the movement of the forehead muscles.
We check whether the forehead tenses when the eyes are opened
and whether the eyebrows rise.
When these two symptoms appear together,
we first consider the possibility of ptosis
as we evaluate the condition.
Next, we directly examine the condition of the eyelid skin
by gently touching it.
If the eyes look noticeably more open when we gently “lift up” the eyelid skin,
this means that the skin itself is pressing the eyes downward.
In such cases, an incisional method involving skin excision
or a forehead lift may need to be considered together
to achieve fundamental improvement.
Conversely, if lifting the skin does not change the size of the eyes,
the issue is more likely to involve the force used to open the eyes
rather than sagging skin.

Finally, we examine whether a double-eyelid crease exists and its shape.
If a partial double-eyelid crease is hidden beneath the skin,
sagging skin needs to be removed.
If there is no crease at all, however, the desired change may be achieved sufficiently
with simple double-eyelid surgery alone.
This evaluation takes only about five minutes at most,
but it can be a decisive step that changes the entire direction of the surgery.
Because there are often multiple causes of a sleepy-looking appearance,
it is important to identify the causes accurately first
and then plan the surgery accordingly.
The causes of a sleepy-looking appearance are more varied than you might expect,
and even people with the same concern may require different surgical plans.
