and every time we receive this question, it weighs heavily on us.
In most cases, the issue is not with the filler itself,
but with the filler having been placed in an inaccurate layer.

In a cross-section of the pelvic area,
[skin → subcutaneous fat layer → fascia → muscle]
are arranged in that order.
For the volume to remain natural and firm,
it must be accurately placed beneath the fat layer,
that is, directly above the fascia.
When the filler is placed in this location,
it is less likely to shift or clump during everyday movement,
and a natural texture that feels like your own tissue can be expected.
To use a simple analogy, just as insulation performs its function
when placed precisely between a wall and wallpaper,

filler also needs to be injected into the correct layer
for its volume and durability to be properly maintained.
If it is placed too superficially, the skin’s surface may become uneven,
while if it is placed too deeply, it may mix with the muscle layer
and feel hard to the touch.
There is one more point we would like to highlight.
The lateral pelvic area contains sensory nerve branches,
including the superior cluneal nerves.
If the injection site is close to the pathway of these nerves,
temporary decreased sensation or abnormal sensations may occur after the procedure,
and in rare cases, the symptoms may persist.
Therefore, approaching the area only after having a thorough understanding
of its anatomical structures can minimize the possibility of these side effects.
Because the thickness of each layer and the location of the nerves vary by body type,
people with slender builds have thinner fat layers, so even a slightly deeper injection
may come close to structures at risk,
while people with sufficient fat have wider spaces between layers,
requiring a different injection plan.

A short duration may not be

The duration depends much more on “how it is injected”
than on which product is used.
Hyaluronic acid filler is gradually broken down
by hyaluronidase, an enzyme in the body.
Unlike the face, the pelvis is an area where sitting, walking, and lying down
are repeated hundreds of times a day.
Therefore, even when the same product is used,
physical pressure may accelerate the rate of breakdown.

That is why the key factor determining durability
is the “layering technique.”
Rather than concentrating the filler in one layer,
it is distributed across several layers, from beneath the subcutaneous fat
to directly above the fascia.
Because pressure is not concentrated in one particular layer,
the rate of breakdown caused by friction tends to decrease,
while the volume is maintained much more evenly.
It follows the same principle as a cake:
if it is built up high in just one layer, it can easily collapse,
but if it is built in several layers, it can remain stable.
We would also like to address a practical question here.

Immediately after the procedure, the volume may
look slightly greater than it actually is.
This is because of the initial swelling,
which usually subsides naturally within one to two weeks.
There may be a heavy feeling on the day of the procedure,
and the discomfort generally gradually improves over the following two to three days.
For about a week after the procedure, it is best to avoid
strong pressure or massage on the treated area,
and reducing the amount of time spent sitting on hard surfaces
as much as possible can also be helpful.
