When searching for philtrum reduction surgery,
you may come across reviews saying,
“the shape of my lips changed” or “my smile became awkward.”
You may desperately want to shorten your philtrum,
but after reading comments like these,
it is not easy to make a decision.
Therefore, in this post, we will honestly explain
why the side effect of unnatural-looking lips occurs,
and how careful surgical planning can reduce concerns about side effects
and address potential disadvantages.
Why does the side effect of
unnatural-looking lips occur?

Quite a few people say the following during consultations:
I had a consultation for philtrum reduction somewhere else,
and they told me that the shape of my lips could change.
So I am wondering whether I should just give up.
Whenever I hear this, I feel regretful,
because once the cause of the side effect is understood,
there are many cases in which giving up is not necessarily required.

The side effect of unnatural-looking lips
is usually related to the location of the incision.
The traditional philtrum reduction technique involves
making one long, continuous incision from the alar base,
along the base of the nose, and beneath the columella.
Although this method has intuitive effects and is convenient to perform,
if the skin characteristics of East Asians are not taken into consideration,
the risk of side effects may increase.

Due to their skin characteristics, East Asians are more likely to develop
hypertrophic scars that become red or raised after suturing.
In addition, the area extending from beneath the nose to the lips
moves constantly whenever you speak or chew food.
As continuous tension is placed on the suture line,
the scar gradually widens, and the scar pulls on the surrounding tissue,
affecting the movement or shape of the lips.
Lip-related side effects are most often
caused by the incision design.
Once you understand this difference,
you will naturally appreciate how important the approach is.
So where and how should
the incision be designed?

Ultimately, there are two keys to reducing concerns about side effects:
hiding the incision where it cannot be seen from the front,
and distributing the tension placed on the surface within the deeper tissue.

Instead of making one continuous incision beneath the nose,
we approach the inner and outer areas separately.
For the inner area, an incision is made inside the nostril floor
so that the scar remains in a concealed location where it is difficult to see.
For the outer area, the incision follows the natural crease
where the nostril wings fold. Since this area is naturally a skin fold,
after healing it appears like part of the crease.

In addition, along with removing excess skin,
we always perform a procedure to lift and secure the inner tissue with sutures.
If only the surface is cut, the pulling force is concentrated on the skin,
but securing the deeper tissue as well distributes the tension
and can reduce the likelihood of the scar widening.

It is also helpful to know the recovery process in advance.
Swelling is usually at its worst on the third day and then gradually subsides,
and the stitches are removed on the seventh day.
You may wash your face starting the next day,
but you should avoid rubbing the surgical area.
Makeup can be applied starting the day after the stitches are removed.
Because the surgical area is sensitive to ultraviolet light during this period,
applying sunscreen carefully for approximately two months
is also important for scar care.
To be completely honest,
philtrum reduction is not suitable for everyone.
If the issue in the middle of the face is caused by the alignment of the teeth
or the structure of the oral cavity rather than the length of the philtrum,
it may be difficult to achieve the desired change with this procedure alone.
This is why we examine the front, 45-degree,
and side views together during the consultation.
First confirming whether you are a suitable candidate
is a wise way to reduce unnecessary trial and error.

Philtrum reduction is a procedure that can significantly change
the overall impression of the face even with a small incision.
However, the results can vary considerably
depending on the surgical design.
It is Globie Plastic Surgery Clinic's principle
to transparently explain the possibility of side effects
and the approach suitable for each individual case.
