It has been three months since my surgery,

and I’m not happy with the shape.

Can I have revision surgery right away?

This is something we hear fairly often in the consultation room.

We fully understand how frustrating this can feel,

but to be honest, at three months,

the body has not yet completely settled.

After breast implant surgery, it generally takes

six months to one year for the surrounding tissue to stabilize.

Before this period has passed, it is difficult to judge

the current shape as the final result.

During the first three to four months, the shape continues to change

as the swelling subsides, and the implants gradually descend

and settle into position.

When we directly examine the breasts of patients

who visit during this period, in most cases the tissue

is still firm or the swelling has not completely subsided.

Just as a new pair of shoes needs time to conform to the shape of your feet,

implants also need sufficient time to blend with the surrounding tissue.

There are many cases in which a shape that initially felt “not quite right”

naturally settles into place after several more months.

Conversely, a shape that looked good at first

may subtly change as the tissue settles.

It is not too late at all to make a decision

after all of these changes have been completed.

For this reason, at Globie, our policy is to discuss the direction

of revision surgery only after allowing sufficient time for stabilization

and directly checking the condition.

The Timing of Revision Surgery Depends on the “Reason”

I developed capsular contracture.

When should I have surgery for this?

Is it different from changing the implants because of the shape?

The criteria for determining the timing of revision surgery

depend on the reason for wanting the surgery.

The starting point is to distinguish whether the reason is elective,

such as dissatisfaction with the shape or size,

or medically necessary, such as capsular contracture or implant rupture.

If the reason is dissatisfaction with the shape,

it is advisable to wait at least six months to one year,

confirm that the final shape has settled,

and then make a plan.

A situation we often encounter in practice

is one that can be resolved without replacing the implants.

In cases of rippling or insufficient cleavage,

correction may be possible with fat grafting alone

to the affected area.

In one actual case, a patient visited us

because an implant had shifted outward.

After harvesting 600 cc from the thighs and obtaining 330 cc

of purified fat, we grafted 180 cc on the right side

and 150 cc on the left.

As a result, we were able to naturally correct the cleavage

and contours without touching the implants.

This is why you do not necessarily need to worry

that the area must be reopened.

Capsular contracture, on the other hand, is different.

Capsular contracture is a condition in which excessive fibrous tissue

forms around the implant, making the breast firm

and potentially causing pain. It is classified into grades 1 through 4.

When examined directly, grades 1 and 2 may feel somewhat firm,

but because there is no major change in shape,

it may be possible to monitor the progress

while combining observation with ultrasound care.

However, if the breast is visibly deformed at grade 3 or higher,

or if pain is present, it may be more appropriate

to make an early surgical plan rather than wait longer

for the stabilization period.

Implant rupture also requires prompt treatment

as soon as it is confirmed.

Ultimately, the key criterion for revision surgery timing

is not “how long you have waited,”

but what signals your body is sending now.

Because breast revision surgery requires much more precise judgment

than the first surgery, the first priority is to accurately understand

your current condition.
