When the front tooth in the middle looks big

Many people who come to the dental clinic
often say things like this.
This time, a female patient in her 20s who visited Gyodae Station Dental Clinic
had the same concern.

Upon checking, the middle front teeth were not actually large;
the lateral incisors were relatively small.
It is called a “microdontia.”
In addition, there were spaces between the teeth,
and the alignment was not straight either.
For those reasons, she even said, “I’m not confident about smiling widely.”
But as I looked into the patient’s mouth,
I had a different thought.

🦷 Another microdontia case
The problem was not the front teeth, but the side teeth!

However, because the lateral incisors were small and set back,
a visual illusion made the front teeth
appear much larger.

The front teeth were slightly rotated and angled outward,
while the side teeth were facing straight forward,
which made the overall alignment look even more crooked.
On the surface, it may seem like a simple issue of tooth size,
but when you look closely, the combination of alignment and angle
was what determined the overall impression.
If you overlook this difference,
the result will always end up awkward.

Focus on harmony while touching the teeth as little as possible!

I explained it to the patient like this.
“Your front teeth are fine as they are.
If we reinforce only the side teeth,
the issue of the front teeth looking too large will be resolved naturally.
First, let’s design the side teeth with resin,
and if you still don’t like it,
we can discuss the front teeth then. That won’t be too late.”

Treatment is not simply about filling empty spaces
and making teeth bigger;
it is about considering the entire dentition, facial features, and smile line
and achieving harmony.
My philosophy is always the same.
Touch less, make it more natural, and let it last longer.

They trusted me and agreed to the treatment,
and I also informed them that after the upper front teeth are completed,
the lower teeth may need adjustment later.
When an ideal side-tooth shape is created,
the lower teeth that had not touched before
may suddenly start touching.
Then, of course, we adjust the inner side of the added
resin (the palatal surface) so that it does not touch,
and if further shaping would make it easy to break,
we may have to adjust the bite of the lower teeth.
I explain this to patients receiving front-tooth treatment as above :)
Small angle differences can make a big difference.

Because the front teeth are slightly rotated,
if you force the side teeth to face straight forward,
it actually looks even more awkward.
So when I place resin,
I often give it an angle so the side teeth look as if they are slightly rotated.
First, I used a rubber dam.

A single detail like this can determine the treatment result 😄
Teeth do not have a single uniform color;
the inner area is a bit deeper, and the outer area is brighter and more translucent.

So I layered the resin, making the inner area darker
and the outer area more translucent.
Just like a painter adding shadows on a canvas
to create depth ~~🧑🏻🎨

After the treatment was finished, the patient held up a mirror
and looked at it for a long time.
Then she smiled brightly and said this.

Doctor, I’m really glad we didn’t touch the front teeth.
Just doing the side teeth made the teeth look even.

In fact, by reinforcing only the side teeth,
the issue of the front teeth looking too large disappeared,
and the spaces between the teeth were naturally filled in.
From the side, the spaces were neatly closed,
but from the front, there was no feeling at all
that the teeth had suddenly become bigger.

That was the moment when the design using visual illusion
worked properly.
Even a small tooth contains
the balance of the face and the naturalness
of the smile.

That is why I avoid unnecessarily shaving teeth
and instead pursue minimally invasive treatment,
designing only the parts that are truly needed.
Instead, I spend time more obsessively than anyone else
on the details and the quality of completion ✌️

Going forward, I will continue designing patients’ smiles
so they are touched less, look more natural,
and can be used for a long time.
As in today’s case, when the concern of
“I’m worried because my front teeth look too large”
turns into “If we restore the side teeth, the front teeth will look better,”
I think that is the realization of the treatment philosophy
I pursue, haha.
