This is a question we hear quite often from patients.
Some people ask whether we only treat front teeth because we love front-tooth treatment so much.. In fact, because we’re so serious about preserving teeth, we seem to approach resin treatment even more actively.
We’re working hard on another conservative treatment, similar in nature to resin veneers.
That is, “delaying root canal treatment.”

Today, a patient came in with a small hole in a tooth, worried a lot that they might “need a root canal,” but we stabilized the pulp without root canal treatment and finished it with resin on the same day of the visit using OOO.

On the outside, it looks fine, but inside it’s already a warning sign

Today’s patient had decay deep inside a small molar.
It looked fine from the outside, but when we checked with a CeraY diagnostic device, the decay hidden deep inside the tooth had reached right up to the pulp.
CeraY is a diagnostic device that shows decay and cracks that are not easily visible to the eye.

Because it displays the activity of decay in color through light reflection, you can see at a glance how far it has progressed even in a tiny cavity.

When we checked with an X-ray, fortunately the inflammation was not severe yet. So we decided to finish it with resin without a root canal.
I tend to delay root canal treatment as much as possible.
Since having root canal treatment means the tooth’s lifespan becomes that much shorter.. as someone who prioritizes conservative treatment, it is not a treatment I particularly prefer.
(*Of course, when decay reaches the pulp and acute pain occurs, I have no choice but to proceed as well ㅜㅜ)

We decided to protect the area where the pulp was slightly exposed with a barrier made of MTA, a biocompatible material, and then restore it with resin, which has strong adhesive strength.
This process is called resin buildup. It is not simply about filling a hole, but about restoring the tooth’s original strength and shape^^

Not every case of pulp exposure requires root canal treatment, and because of the complexity of the root canal system, modern dentistry cannot precisely determine whether root canal treatment has been successfully completed. So I believe it is better to finish treatment in the area visible to the dentist’s eye.
So in this case as well, we decided to protect the tooth through the MTA material and resin buildup process^^

It was enough to proceed even without anesthesia, and we removed the decay cleanly.

At this point, we completely blocked saliva and moisture with a rubber dam to create an environment where the material could bond properly. Then we applied MTA to the pulp exposure site to stabilize the pulp.

MTA란?

This material surrounds the pulp, helps prevent inflammation, and supports the tooth’s own recovery. In simple terms, it sets up a shield of natural healing inside the tooth^^ (I’ll explain this in more detail next time!)

Finally, we built up layers of resin to restore the shape. While considering the direction of chewing forces, the contact point with adjacent teeth, and even subtle differences in height, we completed it with a natural shape.
Leaving the pulp means leaving life

Leaving the pulp means that the tooth can continue to supply itself with moisture and sense feeling.
In other words, it means it functions as a “living tooth.”
