Hello, I am Dr. Park Hyung-seok, a board-certified plastic surgeon.
Recently, I recommended a mini neck lift to a client in their mid-30s who was concerned about a double chin...

As shown in the photo above,
although swelling was still present after surgery,
the double chin had improved, and I can still remember how grateful the patient looked.
There are other similar cases...

In fact, there is a lot of demand for double chin treatment,
but many people regard double chin lifting as a rather intimidating and unfamiliar type of treatment.
So as an alternative,
many choose neck liposuction, which is a much lighter treatment...
but the results often fall short of expectations.
So today,
I’d like to talk a bit about
the mini neck lift surgery
that improves a double chin.
I had double chin liposuction,
but it didn’t work!

During my 10-plus years working as a plastic surgeon,
I have often met people with this kind of concern.
They invariably ask this question.
Doctor, what treatment do you recommend for my neck?
I always recommend a neck lift.
Of course, many people are afraid of surgery and hesitate.
Even though I have extensive experience with jawline liposuction,
I recommend neck lifts more often because of anatomical reasons.
Why neck lifting is more desirable: the structure of the neck
As shown in the figure below,
the neck is made up of multiple layers of tissue.

1 = skin
2 = subcutaneous fat
3 = muscle (platysma, broad neck muscle)
4 = deep fat
5 = digastric muscle
6 = salivary gland
7 = mylohyoid muscle

Double chin liposuction
can reduce the superficial subcutaneous fat
that corresponds to item 2.
However, it has limitations when it comes to suctioning deep fat,
which corresponds to item 4.
This is because it is difficult to control bleeding,
so the muscle in item 3 should not be penetrated.
But neck lifting can safely reduce the structures corresponding to items 4 to 6
while controlling bleeding,
below the muscle.

Because the tissues are treated while being directly seen,
it is much safer than liposuction.

Incision line for a neck lift (red line)

Unless it is performed together with a facelift,
a neck lift is done through a small incision of about 3 cm under the chin,
as shown in the figure above.
Because the incision line is short, it is also called a mini neck lift.
Another advantage is that it is made in a location that is not noticeable.
After the skin incision, some of the deep fat and salivary gland are removed,
and the left and right platysma muscles are brought toward the center and tightened together.
This surgery, also called muscle binding,
briefly became popular in Gangnam plastic surgery clinics.

Muscle tightened in muscle-binding surgery (platysma)
The reason this surgery did not become widespread among many clients
is that simply tightening the muscle alone does not produce
the desired surgical result.

Because this is a clinic specializing in lifting surgery for middle-aged patients,
neck lifting is often performed together with facelifts (link), but recently,
younger patients in their 30s and 40s with double chins
have also been receiving mini neck lifts at our hospital.

The reason a short incision can still be effective is

Why the skin incision can be short in a neck lift (Source: Art of Aesthetic Surgery 2020)
because when a sagging neck like C in the figure above
becomes like D,
more skin surplus is needed.
In other words, because there is actually no skin sagging,
a small incision can still produce good results!
When visiting a plastic surgery clinic to ask about your concerns,
it is most important to find a medical team that can accurately diagnose the cause,
and that has extensive experience in treatments that remove that cause.
