Not long ago,
a familiar patient I had seen before
came to visit me.
This felt like

When the pupil is covered a lot by the eyelid,
it is often called sleepy eyes.

Example of ptosis
The medical term is ptosis.
The treatment for this is eyelid correction (= ptosis correction).
Although it’s not something a plastic surgeon should say^^;;
just because eyes look sleepy, that doesn’t mean every case needs treatment!
Most people would agree that eyes with enough of the pupil showing are pretty eyes,
but!
On the other hand, moderate ptosis
can also become a unique attractive point.

In any case...
an old patient came back to see me.
They had previously undergone incisional eyelid correction and epicanthoplasty.

The photo above shows
the before-and-after results of the eyelid correction surgery.
It was a satisfactory result for male eyelid correction,
but personally, I felt a bit of regret about the undercorrection on the right side,
so it was a case I remembered...
(It was even more memorable because I operated on siblings^^)
Anyway, the patient, who had been curious about the before-and-after appearance of the eyelid correction,
came to see me in person one year after surgery,
and I was very glad to see them.

Deciding on revision eyelid correction!
As expected,
the patient said they liked the left eye very much,
but felt the right side was a bit disappointing.
Specifically...

The goals of the patient and medical team matched, so revision surgery was decided upon
The front line looked less extended (red arrow),
the eyelid correction seemed slightly insufficient compared to the left side (green arrow),
and the end line looked as if it had loosened (blue arrow),
so these were the three things they wanted to improve...
This matched exactly
with the points of regret I had felt
when reviewing the photos in the past,
and with the patient’s wishes,
so we were both happy to proceed with revision.
Plastic surgery means that
even if the medical team feels the result is disappointing,
they cannot force the patient into revision surgery,
(respect for subjectivity)
On the other hand, if the patient’s wishes
fall outside the generally accepted standards of beauty in a given era or culture,
the doctor needs to help adjust those expectations.

The appearance below shows only the right eye
just 8 days after revision eyelid correction...
Not only did the surgery go well,
but the patient also followed the postoperative precautions carefully,
so it seems only a matter of time before the results become even better.

I knew exactly what structures were inside,
so I was confident about the revision.
At the same time, the patient trusted me,
and because they stayed calm without feeling anxious or tense,
I think we were able to create the result both of us wanted even more.
Through this patient’s visit,
I was reminded once again that plastic surgery is a process
created together by the patient and the medical team.
Eyelid Correction Epilogue

but strictly speaking, double-eyelid surgery and eyelid correction are completely different (see link)
Double-eyelid surgery creates a fold line in the eyelid,
