When consulting about implants, many people ask whether they can be used for a lifetime once implant surgery is performed. Today, we would like to explain how long implants can actually be used. To do that, we first need to understand what an implant is.

Did you know that there are many different types of implants?! Even if most people are somewhat familiar with implant types, the first names that usually come to mind are brands such as Osstem, Straumann, and MegaGen. In fact, even if the same brand of implant is placed, the size of the implant used and the surgical method can differ depending on your case.

Also, there is not just one type of prosthetic placed on top of an implant. If you have had implant surgery before, you may have experienced trouble because of the hole in the implant. You might think that an implant you used to rely on suddenly broke and developed a hole, but this is a misunderstanding caused by different types of implant prosthetics rather than the implant itself breaking.

Then shall we first look at the types of implant prosthetics?

Implant prosthetics can broadly be divided into two cases. Let me show you with pictures first.

Can you see the main difference between these two cases?

The biggest visible difference is whether there is a hole in the crown, which is the upper part. The image on the left shows a crown with no hole in the top, while the image on the right shows a structure with a hole in the top of the crown.

The screw-retained crown method on the right is the structure commonly used for implants with a hole. It connects the implant body (artificial root, fixture) embedded in the gum and the upper implant part called the crown, using a yellow screw through the pillar that looks gray in the middle (abutment), thereby linking the three structures: the implant body, crown, and abutment. That is why a hole appears in the crown, which is the upper part. After all three structures are connected, the remaining hole in the crown is filled with a material called resin.

There is a reason why this kind of implant is often used. It is the answer to the question we started with in the title: can implants be used for a lifetime?

Problems after implant placement can largely be divided into three categories:

• When inflammation develops in the implant placed inside the gum

• When a lot of food gets trapped around the implant crown, causing inflammation around the implant crown

• When the implant crown breaks

As such, even with implants, various problems can occur if they are not properly cared for. When inflammation occurs, if the implant is made in the structure divided into multiple parts as shown above, the hole filled with resin can be opened, and only the problematic part can be replaced.

However, in the first structure, where the crown is directly attached to the implant with cement adhesive rather than being connected by a screw as in the second structure, if a problem occurs with the implant, there is the inconvenience of having to remove the entire implant and place a new one again.

However, a prosthesis in the form of a screw-connected crown cannot be used in every case. This is because the appropriate type of prosthesis is selected according to your brushing habits, oral hygiene environment, shape of the jawbone, and systemic health conditions. That is why, when getting an implant, it is important to establish a plan that suits you and perform the surgery according to that plan.
