Among patients who visit for new prosthetic treatment, there are many cases where a crown that was placed a long time ago has come off. Usually, patients come in thinking that the crown can simply be put back on, or that a new crown can be made. However, when we examine the tooth after the crown comes off, there are often cases where the crown portion has decayed extensively and only the root remains.
If some part of the tooth remains above the gumline, there may still be a way to save it. But if only the root remains, or if the inflammation is severe, it is very difficult to preserve the tooth. Even if the crown is placed again, it is unlikely to last long, and because pain or reinfection is likely to recur, if the tooth is judged to be non-restorable, we fully explain the current condition and prognosis to the patient and then proceed with extraction.
One factor considered together with the extraction plan was the timing of implant placement. In general, an implant is placed after waiting a certain period for the gums and bone to heal following extraction. However, if the condition of the jawbone is good and the inflammation is limited to a localized area, immediate implant placement after extraction is also possible.

If the remaining root has already undergone root canal treatment and the caries has progressed significantly, it is very difficult to grasp the tooth with instruments, and it can crumble easily, making extraction highly difficult. Especially when placing an implant immediately after extraction, the key is to preserve as much surrounding bone as possible during the extraction. After carefully removing the root as gently as possible, the inflamed tissue and hidden tartar around the extraction site must be thoroughly removed before the implant is placed to minimize postoperative complications.
The greatest advantage of immediate implant placement after extraction is that it shortens the treatment period. By reducing the waiting time after extraction by several months, the period during which the patient must be without a tooth can be minimized. It can also help prevent rapid resorption of the jawbone to some extent, so if the conditions are suitable, immediate implant placement can be a good option. In addition, because the number of surgeries is reduced, the burden on the patient is relatively lower. After placing the implant, we waited sufficiently for about three months, confirmed that the implant had integrated well with the bone, and then completed the crown.

When a gold crown comes off, it is often not simply a problem with the prosthesis itself. This is because the treatment direction can change completely depending on the condition of the tooth root or gums that are not visible from the outside. In a case like this one, where the tooth can no longer be saved, treatment time and discomfort can be reduced through immediate implant placement after extraction, depending on the condition. However, immediate implant placement is not a treatment that can be applied to every patient. If the inflammation is severe, if there has been significant loss of jawbone, or if recovery is difficult due to systemic disease, proceeding in stages may actually be safer. That is why, rather than simply being a "quickly placed implant," what matters most is an accurate diagnosis and judgment tailored to the patient's condition.
