Today, we would like to introduce a case in which a male patient in his 30s who wanted treatment for a broken front tooth
was unexpectedly found to have a cyst near the root of the tooth,
leading to extraction, cyst removal, and front tooth implant surgery all at once.
My front tooth is broken. I am visiting the dentist for the first time in a long time.
The patient visited our clinic with part of his front tooth broken due to trauma.
At first glance, the tip of the front tooth appeared only slightly chipped,
but to make an accurate diagnosis, we performed panoramic X-ray and CT imaging.

When we examined the inside of the front tooth that seemed to have only a simple fracture,
a large cyst (periapical cyst) had formed in the root area of tooth #11,
and overall, there was extensive tooth damage requiring treatment.
The patient initially wanted treatment only for the front tooth,
so we decided to proceed with treatment limited to the front tooth.
✅ Although it looked like a small fracture on the outside,
✅ extraction was unavoidable internally,
✅ and cyst removal with immediate implant surgery was necessary.

① Extraction of tooth #11 and cyst removal surgery
– At the same time as extraction, the 2 cm cyst lesion was completely removed.
– Allograft bone was also placed in the bone area resorbed by the cyst.
② Same-day implant placement
– After carefully checking the bone condition, surgery was performed at the depth and angle suitable for immediate placement.
– Since this was the anatomically important front tooth area, precise angle and depth control were necessary.
③ Recovery after surgery and waiting for prosthetic restoration
– After surgery, inflammation is controlled and tissue stabilization is encouraged for 2 to 3 months,
– then the treatment proceeds to the final prosthetic fabrication stage.

→ Findings consistent with a periapical cyst
📌 Interpretation: This lesion was judged to be an inflammatory periapical cyst,
which is a representative type of cyst that usually forms at the tip of the tooth root due to tooth infection or trauma.

– It is difficult to determine the condition of a front tooth based only on what is visible on the outside.
In particular, after trauma, the tooth root and gum bone are often damaged as well,

– It depends on the size and location of the cyst and the condition of the tooth.
