The number is not simply a matter of more or fewer teeth.

The person I am introducing today actually had veneers placed on the petite teeth while excluding the central incisors.

The difference is clear when looking closely at the front teeth.

The central incisors are long and wide, while the teeth beside them are short and narrow.

This condition, in which the lateral incisors develop unusually small, is called "peg laterals" or "microdontia."

It is not simply a problem with one tooth; rather, the size balance among the front teeth has been disrupted.

The examination also showed a tendency toward an open bite, with a slight gap between the front teeth when biting.

There was also gum inflammation, so we first cleaned the tartar before proceeding.

Why the central incisors were excluded

In this case, the treatment was planned as petite teeth veneer treatment, excluding the central incisors.

This treatment plan reflected the patient’s wishes,

and we agreed that it was a good approach.

There were no major problems with the size or shape of the central incisors.

Rather than trimming healthy teeth and covering them with ceramic,

it was better to leave them as they were.

Instead, we planned two additional treatments together.

We decided to brighten the central incisors with whitening.

We also performed only minimal tooth reshaping to adjust the tooth axes.

There were three things we focused on in this case.

✅ First, how far the problematic teeth extended.

The treatment area included the small lateral incisors and the line extending beside them.

✅ Second, how far the teeth were visible when smiling.

When smiling broadly, the premolars were also visible,

and dark spaces were forming in those areas.

Therefore, we decided to include the premolars on both sides, for a total of six teeth.

✅ Third, where to place the color boundary.

It is best to place the boundary on the inner side, where it is less noticeable.

Details we focused on in the design

We increased both the width and length of the small lateral incisors.

Attaching ceramic to the front surfaces slightly widens the curve of the dental arch,

creating the impression of increased width.

We preserved the pointed appearance of the canine tips.

Because the canines and premolars are involved when moving the jaw from side to side,

rounding their shape carelessly can change the bite.

The shade was selected to be only slightly brighter than the central incisors.

This was calculated in advance based on the brightness expected after whitening.

To preserve translucency and create a seamless transition with the adjacent natural teeth,

we coordinated the shade with the in-house dental laboratory.

The areas that looked empty because of the small teeth were filled,

and the entire front teeth formed one continuous line.

The untouched central incisors and the six newly created teeth

do not look disconnected from one another.

Q. Does the cost increase as the number of teeth increases?

Because the cost is calculated per tooth, it increases as the number of teeth increases.

However, treating teeth that do not need treatment means sacrificing more natural tooth structure. As in this case, excluding teeth that can be left untreated is also better in terms of cost.

Q. If the central incisors are not treated, won’t the color stand out?

That is why we also establish a whitening plan.

By selecting the ceramic shade based on the brightness after whitening,

the boundary does not become noticeable.

Q. Do a lot of natural teeth need to be shaved down?

Because the lateral incisors are naturally small, the amount of reduction is actually relatively limited. Since the treatment involves making them larger, there is less need to create additional space.
