One of the most common questions patients ask after a hair transplant is, “When can I wash my hair?”
Many people worry that the follicles may be damaged or fall out during the process of cleaning the transplanted area.
However, if you wash it the right way, it can help prevent infection and support faster recovery.
Based on recently published research findings, here is the scientific basis for washing your hair with confidence after a hair transplant.

A research team from the Department of Plastic Surgery at Hanyang University conducted a study to verify whether wound cleansing with tap water and commercial soap is safe and effective outside the hospital.
Instead of the conventional method using normal saline and antiseptics,
they evaluated how cleansing with ordinary tap water and surfactant-containing cleansers (Decyl glucoside, Sodium C14-16 olefin sulfate) affected infection prevention and wound healing.
Experimental method
o Postoperative wounds: 9 patients (including plastic surgery)
o Trauma (lacerated wounds): 4 patients
o Infectious wounds: 2 patients
o Wounds washed once a day with tap water and commercial soap (for at least 5 minutes)
o Protected with foam dressing after washing
o No separate antiseptics or normal saline used
- Follow-up: Infection status, dehiscence, and healing speed evaluated over 2 weeks
Results
• All patients’ wounds healed completely within 2 weeks
• No cases of infection, delayed healing, or dehiscence occurred
• The same results were confirmed even in surgical wounds on the face and scalp
• Compared with conventional normal saline, there was no difference in infection rate or healing speed
This study shows that wound cleansing using tap water and soap containing surfactants has an effect equivalent to the saline-based cleansing method used in hospitals.
Hair washing after a hair transplant: why you can feel reassured
Many people worry that washing their hair after a hair transplant might harm the transplanted area.
But based on the study above, if you clean it properly, it can actually help prevent infection and support faster recovery.

◆ It is okay to wash your hair starting the day after surgery
• You can wash your hair starting the day after a hair transplant.
• However, you should be careful because rubbing the transplanted area too hard may cause follicles to come out.
• Until about 2–3 days, before scabs form, keep it light and only wash gently by applying foam.
◆ Will the follicles fall out?
• During the initial 3–5 days after a hair transplant, you should avoid excessive irritation, but gentle washing does not affect the follicles.
• The study also confirmed that there were no cases of dehiscence after washing.
◆ Is it okay to wash with tap water?
• Washing with tap water showed the same effect as using normal saline, and the risk of infection did not increase.
• In other words, if used properly, tap water can also be used safely.
◆ Is it okay to use soap?
• The commercial soap containing surfactants used in the study was proven to help prevent infection without negatively affecting the healing speed.
• However, it is better to avoid using strong cleansers or products containing alcohol.
Proper hair-washing method after a hair transplant

✔ Day 1 after transplantation
• You can wash your hair starting the day after surgery.
• However, the transplanted area should not be rubbed hard; it should be cared for gently, with only foam applied.
✔ Days 2–3 after transplantation
• Lightly moisten the area by spraying normal saline or tap water with a spray bottle.
• Do not rub with your hands; let it be rinsed away naturally with running water.
✔ Days 4–7 after transplantation
• Use lukewarm water and a mild shampoo to gently create foam, then wash by lightly patting with your fingertips.
• Never scratch with your nails or rub hard.
✔ Weeks 1–2 after transplantation
• As in the study results, normal washing using tap water and a mild cleanser is possible.
• Gently massaging the scalp to stimulate it and help blood circulation is also good.
✔ Things to keep in mind
• Avoid hot water (risk of follicle damage)
• Avoid high-pressure shower spray
• Instead of air drying, it is better to gently pat dry with a soft towel
