There’s something I hear very often in the consultation room.
“Doctor, I thought my eyes were small.”
“Maybe I just look tired because I haven’t been sleeping well?”

But after consultation,
quite often, it turns out the eyes are not small at all, but rather appear sunken.
Today, I’ll explain in a simple way:

✔ Why they can make you look older and tired

👁️ What Are Sunken Eyes?
Sunken eyes do not mean the eyes are actually small.
They refer to a condition where the volume above and below the eyes decreases, making the eyes look like they have receded inward.
In particular, these features often appear together.

- The area above the eyes looks hollow

- The double eyelid crease looks higher

- The space between the eyes and eyebrows looks wider

- Shadows make the face seem tired all the time

- Looks more mature than your age, or gives a premature-aging impression
In photos,
👉 the eye area does not look clear, and the hollowed-in feeling is commonly visible.
🤔 Why Sunken Eyes Make You Look Older
The eyes are one of the first areas on the face to show signs of aging.
When you have sunken eyes,
- Loss of volume above the eyes makes the bone structure stand out more
- Shadows form around the eyes, creating a constantly tired impression
- Your expression may seem strong or even cold
That’s why, compared to your actual age,
👉 it is not uncommon to look 5 to 10 years older.

- Your double eyelids have become deeper than before

If 2 or 3 or more of these apply,
the issue is likely not just the eye shape, but a volume issue.
🧑⚕️ Are Sunken Eyes Always Treated with Double Eyelid Surgery?
No.
If you only adjust the double eyelid on sunken eyes,
they may end up looking even more hollow, or the impression may become stronger.
What matters is
not the double eyelid itself,
but how the volume around the eyes is restored.
So during consultation, you need to look at
- The degree of hollowness above the eyes
- The distance between the eyebrows and the eyes
- Skin thickness
- Overall facial volume
✔ How We Approach Sunken Eye Correction
It varies by case, but in general,
Supplementing volume above the eyes (fat grafting)
If needed, contouring the crease with non-incisional double eyelid surgery
If hollowness is present together with under-eye bulging, combining this with lower eyelid fat repositioning
👉 Rather than “making the eyes bigger,”
it is closer to the idea of “making the eyes look clearer and more defined.”
“My eyes look so tired—why is that?”
“Why do I suddenly look older?”
In many cases, the cause is not eye size, but “hollowness.”
Sunken eyes are
✔ something that can be improved sufficiently
✔ possible to address without overly extensive surgery
✔ a feature that can significantly change your overall impression
