"After the first surgery, my eyes became too thick and unnatural. Can this really be improved?"
"Only one eye loosened, so my eyes became uneven. Can revision surgery restore symmetry?"
Hello. I’m Lim Seong-yoon, the director of Just Plastic Surgery and a board-certified plastic surgeon with 21 years of experience.
Over the past 20 years, I’ve met countless patients seeking double eyelid revision surgery. What I’ve come to realize is that even though it is all revision surgery, the approach for correcting “sausage eyes” is completely different from the approach for correcting uneven eyes.
If you undergo surgery without fully understanding this difference, you may not achieve a satisfactory result even after the second operation.
Today, I’ll explain what kind of approach is needed for each representative type that requires double eyelid revision surgery, and why a customized plan that matches the type is so important. Please take just 3 minutes to read carefully.

Sausage eyes: You first need to understand why they happen
Sausage eyes, where the eyelids look unnaturally thick and bulky after the first surgery, usually result from excessive tissue removal or incorrect line design.
If too much eyelid fat is removed or the muscle layer is excessively excised, the eyelids can look swollen and thick. The same problem can also occur when the double eyelid line is set too high or too wide for the person’s eye structure.
In revision surgery for sausage eyes, the key is first to lower the existing line or adjust its width. At this stage, the condition of the remaining eyelid tissue must be precisely assessed. If there is damaged tissue, it must be restored while creating a natural-looking line at the same time, which requires advanced skill. If there is insufficient skin, a special technique called dual-line method may be needed, so various approaches are required.

Based on my background in microscopic surgery, I safely restore damaged eye structures and create natural results even with limited tissue by "preserving as much of the existing tissue as possible." We provide detailed preoperative consultation with the director and close postoperative care from our manager, so you can recover with peace of mind even after revision surgery.
Q. Can sausage eyes be improved with revision surgery?
👉 Yes, they can. However, rather than simply lowering the line, good results are possible if surgery is performed after accurately evaluating the condition of the remaining tissue.
Uneven eyes: What are they?
Uneven eyes refer to cases where the degree of black iris exposure differs because the strength of eye opening is different. There are two possibilities: the eyes were already uneven before surgery, but that was ignored during the operation; or unevenness developed after surgery as an acquired condition. Detailed consultation is needed, and accurate assessment, explanation, and precise correction suited to the case are essential before and during surgery.
Asymmetrical double eyelids refer to cases where only one line has loosened, or where the line height on both eyes has formed differently. In such cases, the structures of both eyes must be analyzed accurately and the lines properly matched.
Human eyes are not naturally perfectly symmetrical. Eyelid thickness, fat volume, muscle strength, and even the habit of opening the eyes all differ, so symmetry is not achieved simply by matching the lines.
In double eyelid revision surgery, the loosened side must be re-fixed while the balance with the opposite eye is delicately adjusted. At this point, ptosis (eye-opening strength), the degree of eyelid droop, and even differences in eye size between the left and right sides must all be considered together for natural symmetry to be achieved.

Especially in high-difficulty double eyelid revision surgery, the surgeon’s experience and delicacy determine the outcome. For 21 years, we have performed all eye-related surgeries such as double eyelids, brow lift, upper blepharoplasty, and lower blepharoplasty, and we provide direct pre- and postoperative explanations through 1:1 doctor-responsible care.
Q. Can uneven eyes become completely symmetrical with revision surgery?
👉 Perfect 100% symmetry is difficult, but by carefully analyzing the dominant eye, the problems from the previous surgery, and any pre-existing congenital issues, a natural balance is entirely possible.
A revision plan that suits you is the key
Double eyelid revision surgery is much more difficult than the first surgery. Because it deals with tissue that has already been damaged once, an accurate approach by type is needed, and above all, a customized plan that matches the condition of your eyes is important.
The surgical method changes completely depending on whether the issue is sausage eyes, uneven eyes, or ptosis. So if you are considering revision surgery, I recommend consulting at a clinic that can consider not only the line itself but also the overall balance and function of the eyes.
Thank you for reading to the end. I hope this post helps those considering double eyelid revision surgery.
What I discussed today is a summary of general guidelines. In reality, the approach may differ depending on the individual eye structure, the method used in the first surgery, and the condition of the tissue. To give you an accurate answer, we need to check your specific condition in person, so please upload photos or visit us. Thank you!
