Neck Wrinkles? Differences in Causes Between Horizontal and Vertical Lines, Treatment Types, and Care Methods: A Complete Guide
Hello.
I am Yoo Hyo-seop, a dermatologist with 23 years of experience.
When I see patients, many people continue to manage facial elasticity 꾸준히, but only start worrying about neck wrinkles later.
“Can neck wrinkles be lifted, too?”
“Will Botox help?”
I get all kinds of questions like these.

The neck is an area that is often cared for later than the face, and at the same time, it is also an area where aging appears more quickly.
Especially with neck wrinkles, rather than seeing them as just one problem of “wrinkles appearing,”
the causes and approach differ depending on whether they are vertical wrinkles or horizontal wrinkles.
Today, I will organize and explain everything from the causes of horizontal and vertical wrinkles to the types of treatments and care methods.
Why Neck Wrinkles Appear Before Facial Wrinkles
The neck has structurally weaker conditions than the face.
First, the skin is thinner and there are fewer sebaceous glands, so it dries out easily.

Also, in daily life, people carefully apply sunscreen to their face, but relatively few people pay as much attention to the neck.
In this way,
thin skin
low sebum production
frequent UV exposure
When these three factors overlap, the neck naturally becomes an area where elasticity loss and wrinkles progress more quickly.
As aging progresses, the skin gradually becomes thinner, rougher, and drier, making wrinkles stand out more clearly.
In this process, the vertical and horizontal wrinkles we commonly see appear in different forms.
Vertical Neck Wrinkles Are Greatly Influenced by Muscles

Vertical neck wrinkles are related to a broad muscle called the platysma.
As skin elasticity decreases and this muscle becomes more prominent toward the front, it can appear as two or three band-like lines.
In this case, it is not just a skin issue; tension and movement of the muscle also affect the wrinkles, so the approach changes.
In general, Botox can be applied to a specific area to relax muscle tension,
or, if a more natural improvement is desired, a skin Botox method that is evenly injected over the skin can be considered.

Also, recently, rather than simply improving wrinkles, methods that combine Rejuran and Botox are being used to improve the overall skin condition as well.
In this case, along with wrinkle improvement, it may also help improve dryness and rough skin texture, so it can be useful as a preventive approach.
Horizontal Neck Wrinkles Should Be Approached as a Volume Issue
On the other hand, horizontal neck wrinkles appear through a completely different mechanism from vertical wrinkles.

These wrinkles are less a simple “folded wrinkle” and more a structural issue created by repeated depressions and protrusions.
Therefore, the approach should not be about “pulling” but about “filling.”
Typical examples include:
filler treatment that adds volume to sunken areas
Sculptra, Radiesse, and others that induce the body’s own collagen production

Fillers have the advantage of immediate volume improvement, but in some cases, they may look lumpy or their effect may decrease over time.
On the other hand, Sculptra and Radiesse work by naturally inducing collagen production,
so the result is relatively soft in appearance, but it takes time for the effect to show.
Recently, to compensate for these drawbacks, collagen booster treatments have also been used, expanding the options in terms of longevity and stability.
As such, horizontal neck wrinkles are not something that should be addressed with just one simple treatment; an approach that considers skin condition and structure is important.
Daily Care Matters as Much as Treatment
Neck wrinkles are not completely managed by treatment alone; daily habits must be maintained together for the results to remain stable.
The most basic steps are moisturizing and UV protection.
It is good to use enough moisturizer on the neck as well,
and if you spend a lot of time outdoors, applying sunscreen every 2 to 3 hours can help.

It is also a good idea to physically cover the neck.
Using a mask or neck guard can help reduce UV exposure.
In today’s environment, where smartphone use is frequent, it is easy to repeatedly adopt a posture with the head bent down.
This posture can deepen neck wrinkles and also place strain on the cervical discs, so
it is better to develop the habit of using devices with your gaze slightly raised if possible.
Neck Wrinkles Require an Approach Tailored to the Cause
Neck wrinkles should not be viewed as just one problem; depending on whether they are vertical or horizontal, they require completely different approaches.
If repeating the same treatment has not brought much change, it may be because the cause was approached in the wrong way.
Accurately understanding your current condition and choosing a method suited to the skin and structure are important factors that determine the outcome.
If you are concerned about neck wrinkles, rather than deciding on a treatment type first, I recommend getting an accurate diagnosis of your current condition.
