
An APEC invitation lecture on treatment using Clarity2
Hello,
I’m Choi Won-woo, a dermatologist with 23 years of experience.
Recently, I gave an APEC invitation lecture on treatment using Clarity2,
and I had the opportunity to once again organize my treatment direction based on real clinical experience and data.
When I see patients in the clinic,
there are many cases that cannot simply be explained as “there is pigmentation” or “the acne is severe.”
In particular,
recurrent male melasma
severe acne that does not subside despite inflammation
These two are representative cases that cannot be resolved with a simple procedural approach.

So today, I will share a more realistic treatment direction centered on what I consistently do in actual clinical practice:
Clarity toning (male melasma) and Clarity Gold PTT (refractory acne).
- Male melasma cannot be treated with simple pigmentation treatment alone.
Male melasma has several clear differences compared with female melasma.
Pigment is often distributed more deeply and widely,
and repeated skin irritation from shaving, ultraviolet exposure, lifestyle habits, and other factors creates a structure with a high likelihood of recurrence.

That is why an approach that simply tries to remove pigmentation with strong energy can actually lead to worsening pigmentation.
In this case, one thing is important.
It is not “how strongly to remove it,” but “how stably to reduce it.”
- Clarity toning – a treatment that distributes stimulation
The core of Clarity2 toning is a “short-pulse segmented method” based on a 755 nm Alexandrite laser.

As you can see in the reference image,
it is designed to deliver energy in stages by dividing it into 0.1 ms / 0.3 ms / 0.5 ms.
The reason this method is important is clear.
Instead of delivering strong energy all at once,
it stimulates the pigment in stages, reducing skin damage while allowing only the pigment to respond selectively.

In actual clinical practice, another advantage is that redness immediately after the procedure is minimal,
return to daily life is quick, and repeated treatments do not disrupt the skin’s condition.
In addition, Clarity2 automatically controls skin temperature within a certain range through a “real-time temperature monitoring system.”
This feature is a key factor in reducing the possibility of pigment worsening caused by excessive heat.
- What I feel in clinical practice is the “direction,” not the “speed.”
Looking at the clinical cases in the presentation materials,

After about five treatments, at the 12-week mark,
pigmentation gradually lightens and
an overall more even skin tone appears.
The important point here is,
not a dramatic change after one session, but a steady improvement that does not collapse.
In fact, when treating melasma, it is far more important to create a structure in which it does not return than to make it improve quickly.
- Severe acne is not simply an inflammatory problem.
The reason acne keeps recurring is not simple.

Sebaceous gland hyperactivity
Inflammatory response
Vasodilation
Bacterial environment
These four factors act at the same time, continually worsening the skin condition.
That is why there are many cases in which simple extraction or medication has limitations, and the pattern eventually repeats with recurrence.
- Clarity Gold PTT – changes the acne environment itself.
Clarity2 is used not only for pigmentation treatment, but also for active acne treatment through the Gold PTT method.
The core of this treatment is
using heat and light energy to change the “environment in which acne develops” itself.

Specifically, it suppresses sebaceous gland function,
reduces inflammatory responses, and stabilizes dilated blood vessels.
Even in actual clinical cases, a change in which red inflammation and skin tone stabilize at the same time can be confirmed after just two treatments.
In particular, for recurring inflammatory acne, this approach produces stable results.
- One device can solve different problems in different ways.
The biggest advantage of Clarity2 is not simply its performance,
but the fact that it can be used in completely different ways depending on the purpose.

When these three factors are combined, treatment that produces predictable results becomes possible.
In closing,
Male melasma and severe acne are conditions that are easy to worsen and recur when approached with the idea that they will improve simply because they are treated strongly.
What matters is accurately understanding the current skin condition,
and designing a repeatable treatment structure while minimizing irritation.
Clarity toning and Gold PTT,
when approached in this direction, are methods that can create clinically meaningful changes.
Not just a simple procedure,
but “creating a flow in which the skin steadily improves” is the essence of treatment.
