“Does Ulthera hurt a lot?”
“Can’t I have it done under sleep anesthesia?”

Ulthera is known for producing noticeable lifting effects, but it is also a treatment often associated with concerns about pain.
That is why today I would like to discuss whether sleep anesthesia is truly necessary for Ulthera, how the pain can be managed, and in which cases Ulthera Prime Fit may be considered.
Ulthera is a representative ultrasound lifting treatment sought by many people who want to define their facial contours without surgery.
It can be used to tighten deeper structures, particularly when the jawline has become less defined, the cheeks have started to sag, or a double chin appears heavy.
However, with treatments that can produce good results, the important question is not simply “how strongly should it be performed?”
It is also necessary to assess which layer of the face has sagged, which areas require energy, and how the pain will be managed.

- The reason Ulthera is effective lies in its depth
Ulthera continues to be chosen among lifting devices because of the depth at which it works.
Ulthera delivers high-intensity focused ultrasound energy to the deeper layers within the skin.
It focuses energy near the SMAS layer, which is closely associated with facial sagging and is the fascial layer that supports the facial structure, thereby inducing a tissue-contraction response.
Rather than simply pulling the surface of the skin, it works more like tightening the structures that support the face from within.
Therefore, it can be a more meaningful treatment for people with sagging in the deeper layers, such as cheek sagging or a weakened jawline, rather than for those whose only concern is reduced elasticity on the skin’s surface.

Many people also ask about the number of shots.
Even 100 shots may help with a localized concern in a specific area, such as the center of a double chin.
However, if you want to lift sagging across the entire face, a sufficient number of shots—usually more than that—is generally needed.
That said, the number of shots is not the only important factor.
Even with the same 400 or 500 shots, the results may differ depending on which areas receive energy, at what depth, and in which direction.

Ulthera pain is somewhat different from a simple prickling sensation on the surface of the skin.
Because the ultrasound energy is delivered to deeper layers, some people may feel a sharp, tingling sensation, an aching feeling, or a sensation that seems to radiate toward the teeth, depending on the area treated.
However, this pain does not have to be endured unconditionally.
Because pain is a sensation transmitted through the nerves, it can be managed to a considerable extent by identifying the points where pain is likely to be felt more strongly and applying local anesthesia appropriately.

In many cases, the procedure can be performed comfortably enough using numbing cream, local anesthesia, and adjustments to the energy and speed based on the patient’s response during treatment.
In fact, when patients are awake, it is possible to immediately check whether a particular area is especially uncomfortable or whether the pain feels intense, allowing for more precise adjustments.

- When is Ulthera Prime Fit appropriate?
Ulthera Prime Fit is not the name of a separate device but a combination treatment that uses Ulthera together with fitting lifting.
While Ulthera tightens structures in the deeper layers, near the SMAS, fitting approaches the area by addressing the lower dermis, upper fat layer, and loose skin above it together.
For some people, Ulthera alone is sufficient.
In particular, if the face has volume and sagging in the deeper layers is the primary concern, satisfaction may be high with Ulthera alone.
However, for people whose skin itself is overly soft and lacks firmness, or whose surface skin may remain loose even after fat is reduced, considering fitting in combination may be helpful.

In other words, Ulthera Prime Fit may be suitable for people who need to consider both lifting the deeper layers and tightening the skin.
However, Prime Fit is not necessary for everyone.
Depending on the direction of facial sagging, skin elasticity, fat distribution, and sensitivity to pain, Ulthera alone may be better, or Prime Fit may be more appropriate.
Ultimately, what matters is not the name of a popular treatment but first determining which layer of your face is sagging.
