Many people who have undergone hip fat grafting are worried because the back contour does not improve as much as expected.
Even though the volume seems to have been added, people often say that the side line or the line under the hip feels vague.

This is not simply a matter of how much fat is added, but an area where design, distribution, and the surrounding structure must all be considered together.
Today, I will explain one by one why this difference happens and the key points that are easy to miss.

- Why there is volume but no line definition
Hip fat grafting is not a procedure that simply creates a pretty curve by filling in fat.
In reality, “where and how much is placed” matters much more.

If the volume is concentrated only in the center, the side line may look wider, or the boundary under the hip may appear blurred.
In that case, rather than giving the impression of a lifted back contour, it can simply look like added thickness.

In particular, if the area above the pelvis and the side line are hollow and only the center is filled, the overall proportions can look awkward.
- The key structure that determines the hip line
The back contour is not completed by the hips alone.

The width of the pelvis, the curve connected to the waist, and the descending line at the lower hip all work together.
Especially when the line under the hip is sagging or the boundary is unclear,
upper volume alone does not easily create a lifted impression.
Also, when the side hip is hollow, the center volume becomes more emphasized, which can actually make the line stand out more.
- The distribution design that matters in fat grafting
The result changes greatly depending on which layer the fat is placed in and in what direction it is distributed.

Left) Before, Right) 2 months after grafting
If it is concentrated only in the shallow layer, the surface may look uneven.
If it is filled mainly in the deep layer, there may be volume, but the line may not appear clearly.

In addition, symmetry on the left and right and the balance of curves are also important factors.
That is why it is necessary to divide the work into multiple layers and place the fat in a three-dimensional way, and design tailored to the body type is important.
- Detail points that create differences in results
One of the most commonly overlooked parts is arranging the surrounding lines. If only the hips are filled and the waist or flank lines remain the same,

there may be no contrast, and the back contour can look flat.
On the other hand, if the surrounding lines are also refined, the same volume looks much more three-dimensional.
Another important factor is the retention rate of the grafted fat. If the initial volume is not maintained, the line may become less distinct over time.

In the end, hip fat grafting is not simply a matter of quantity. Satisfaction differs when structural understanding, planning, and detailed adjustments all come together.
The back contour is completed by small differences that greatly affect the overall finish.
