When people come in for consultations because they feel their face looks long,
it is often not just the length itself that is the issue, but a combination of volume deficiency and imbalance in proportions.
In particular, when the midface or cheeks look hollow, the vertical length often appears even more pronounced.

At this point, people often consider fat grafting,
and ask questions such as:
“Will it make the face look shorter?”
“How long will it last?”
For a long face, fat grafting requires an approach that redesigns proportions rather than simply adding volume.

- Structural causes of a long-looking face
The reason a face looks long is not simply because of the jaw length.

Among the proportions of the forehead, midface, and lower face, if the middle area is hollow or lacks volume,
the vertical length is emphasized even more.
In particular, when the front cheekbone area or cheeks are hollow, the center of the face can look empty,
creating an impression that it looks longer overall.

So improving a long face is not about simply reducing length,
but about filling in areas lacking volume to shift attention and balance the proportions. In this process, fat grafting becomes one option.
- Key before-and-after changes from fat grafting
When volume is added to the front cheekbone area, cheeks, and temples through fat grafting,
the horizontal proportion of the face becomes more pronounced,

which creates the effect of the face looking relatively shorter vertically.
In other words, the actual length does not decrease; rather, the proportions are adjusted, changing the overall impression.
Looking at before-and-after changes, the face tends to look less long,
and a flat impression changes into a more three-dimensional one.

In particular, when smiling, hollow areas are often less noticeable, and the overall impression becomes softer.
- Difference between maintenance period and survival rate
With fat grafting, not all of the injected fat remains; some is absorbed and some survives and stays in place.

In general, the volume may initially appear larger due to swelling,
but over time it settles naturally.
The fat that survives tends to last for a relatively long time, but the survival rate can vary depending on an individual’s constitution and blood flow.
For that reason, instead of fully settling after just one procedure, additional touch-up treatment may be considered if needed.
- Design criteria for natural-looking results
When performing fat grafting for a long face, the key is not simply to add a large amount,

but to decide where to distribute it and how much to place in each area.
In particular, the front cheekbone area, cheeks, and temples should be filled in a balanced way to achieve improved proportions without looking excessive.
Also, if the procedure is done without considering skin elasticity and the person’s existing face shape, it can look unnaturally swollen,
so

it is important to design the treatment based on overall harmony.
In improving a long face, fat grafting should be approached as “proportion adjustment,” not just “volume addition,”
which leads to more satisfying results.
