Invisalign Extraction Orthodontics for Protruding Mouth: Are the Results Really Bad?

Today, we would like to introduce a case of extraction orthodontic treatment for a protruding mouth using Invisalign.

When consulting with patients, there is a question we hear very often.

Today, I’m writing this post to break the misconception that extraction orthodontic treatment with Invisalign does not produce good results.

One of the things I often hear during the initial consultation is, “I want to retract my mouth with extraction orthodontic treatment, but when I consulted another clinic, they said Invisalign wouldn’t work.”

In fact, there are still many people who think:

“Clear aligners are only suitable for simple orthodontic cases.”

“Extraction orthodontic treatment is better with braces and wires.”

However, the Invisalign system and orthodontic protocols have advanced tremendously.

Above all, what matters most is not the appliance itself, but the treatment plan and the ability to control the treatment along the way.

Why is Invisalign extraction orthodontic treatment considered difficult?

The biggest reasons people say Invisalign extraction orthodontic treatment is difficult are as follows.

1. Molars can tip into the extraction space easily During the process of moving the teeth backward,

the molars may tilt toward the extraction space.

2. The front bite can become deeper As the front teeth are moved backward,

overbite may become more severe or the bite may not fully settle.

If wear time is insufficient or adaptation to the appliance is delayed,

the teeth may not move as planned.

So today, I’d like to talk about a recently completed Invisalign extraction orthodontic case for a protruding mouth.

In terms of both treatment period and results, I will introduce an Invisalign extraction case for a protruding mouth that is in no way inferior to ordinary Clippy-C metal bracket treatment.

This patient came for an orthodontic consultation in January of last year because of a protruding mouth.

The front teeth are protruded,

and the teeth are not aligned evenly.

Because the patient wanted Invisalign treatment, we extracted the four first premolars and started orthodontic treatment.

After ordering Invisalign clear aligners, the appliances arrive in about a month and a half.

Treatment began in February 2023 after starting to wear the aligners.

More than 80% of the extraction space has been closed.

This is nine months after treatment began.

And this is the appearance after wearing all 47 steps of the first set over 11 months.

With Invisalign, patients usually visit the clinic every two months, and at those visits the doctor checks whether any attachments have come off and whether the teeth are moving properly.

If tooth movement is not occurring as planned, parts of the aligner will no longer fit well.

This patient also had a period during treatment when the aligners were starting to fit poorly, so we extended the replacement interval from the usual 7 days to 10 days.

Looking at the overall course of treatment, it progressed well without any major issues.

The extraction spaces are fully closed,

but there is a slight open bite in the molar occlusion.

To improve the completeness of the treatment result, we proceeded with making additional appliances.

Starting in January 2023, the patient wore additional appliances for about five months, and treatment was finally completed this July.

and by correcting the open bite from the additional appliances, the occlusion—which many people worry about—is also good.

The total treatment period was 1 year and 7 months, less than two years.

Because the lips were relatively thin at the beginning, the treatment plan was designed so that the front teeth would not be retracted too far, and the lip profile after orthodontic treatment is also very good.

The protruded and flared front teeth became neatly aligned in a straight line.

The root parallelism of the teeth is also good.

In Invisalign extraction orthodontic treatment, the molars can easily tip toward the extraction space.

This is important not only at the treatment planning stage, but also through close monitoring during treatment.

In every treatment, moderation is important.

A look where the front teeth are pulled too far back is something people tend to avoid these days.

In particular, caution is needed because it can make the nasolabial folds look deeper or create a peanut-shaped face.

There is no orthodontic treatment that cannot be done with Invisalign.
