Hello. We are Yonsei Baro Dental Clinic, an orthodontic clinic in Apgujeong.
Today, we’ll talk about how to address the black triangle issue in the front teeth, which many people worry about.
Previously, we introduced a case in which black triangles were improved with Clippy C front-tooth partial orthodontic treatment,
and after that, we received many questions.
“Does black triangle treatment have to be wire braces?”
“Can it be done with clear or invisible appliances too?”
So today,
we’ll introduce a case in which black triangles were improved along with re-treatment orthodontics using 2D lingual partial orthodontics that is barely visible from the front.

23.04~23.08
This patient visited for re-treatment because, after removing the wire retainer that had been kept in place after orthodontic treatment long ago, the front teeth had shifted again and become misaligned.

23.04
If you remove the wire retainer several years after orthodontic treatment, relapse is 100%.
However, it varies somewhat depending on the gum condition.
Some people relapse quickly, while others show little shifting even after several years.
From the front, the black triangle was particularly large, and the two front teeth looked especially big.

23.04
Teeth strictly follow perspective.
When patients say that the two front teeth look big, it is often not a problem of tooth size but because the two front teeth protrude more than the adjacent teeth.
If the alignment is corrected, the front teeth looking large is naturally resolved.
Since this was re-treatment, the patient chose the invisible 2D appliance.

23.06
This is the appearance two months after starting treatment.
The 2D appliance is attached to the lingual side at the back, so it is not noticeable from the front that orthodontic treatment is in progress.

23.06
The black triangle has already been reduced quite a bit.
The blue arrow in the photo indicates a device that prevents space from opening behind the canine.
Front-tooth partial orthodontics may look simple, but it is actually a treatment that requires a fair amount of effort to prevent side effects such as space opening behind the canine.

23.08
After that, the re-treatment was completed with two more months of finishing treatment.
The total treatment period was 4 months.
Some people ask whether a short orthodontic treatment period puts a strain on the gums. Teeth do not move quickly just because more force is applied.
Rather, when tooth movement is slow, it places a burden on the gums.
For orthodontic treatment to finish quickly, treatment that predicts all tooth movement patterns in advance and prevents problems from the start is needed so that treatment does not become prolonged.
Ultimately, this requires a great deal of experience and know-how from the orthodontist performing the treatment.

23.04~23.08
From the angled side view, you can also see that the black triangle has been greatly reduced.

23.04~23.08
While performing interproximal reduction and using elastic bands together, we moved the protruding front teeth backward quite a lot.

23.04~23.08

23.04~23.08
The wire retainer was also placed neatly without any foreign-body sensation.
Retainers also need periodic maintenance.
This time, the patient decided to receive regular checkups and thorough maintenance.

23.04~23.08
The problem of the two front teeth looking especially large was also neatly resolved.
It feels good to see a cleaner appearance.
High-difficulty full orthodontic treatment that takes 1 to 2 years is meaningful, but the rewarding part of front-tooth partial orthodontics is that it can produce good results in a short period.
For black triangles, resin or veneers are not the only answer.
If the cause of the black triangle is
an issue with tooth axis,
a problem with alignment,
or a gum issue,
then after an accurate analysis,
partial orthodontics alone can often achieve very good results.
