Hello.
Today, I would like to introduce Invisalign treatment for a protruding mouth.
I am writing this post to dispel the misconception that extracting teeth for orthodontic treatment with Invisalign does not produce good results.
One question I often hear during initial consultations is: “I want to retract my lips through extraction orthodontics, but another dental clinic told me that Invisalign is not an option. Is that true?”
Q. Is extraction orthodontic treatment possible with Invisalign?
A. Yes, it is.
In the past, there was a perception that fixed braces were more advantageous for extraction orthodontics. However, today, if the treatment plan and control are precise, Invisalign can also achieve sufficiently good results.

That is why I would like to share the story of a recently completed Invisalign extraction treatment for a protruding mouth.
I would like to introduce an Invisalign treatment for a protruding mouth that was in no way inferior to conventional Clippy-C fixed braces, both in terms of treatment duration and results.
Q. Why is Invisalign extraction orthodontics considered difficult?
A. Because the extraction spaces must be closed precisely.
In particular, the molars may tip toward the extraction spaces, or problems such as an open bite may occur. Therefore, careful monitoring throughout treatment is very important.

23.01
This patient came in for an orthodontic consultation last January because of a protruding mouth.

23.01
The front teeth were protruding,

23.01
and the teeth were not evenly aligned.
Because the patient wanted Invisalign treatment, the four first premolars were extracted and orthodontic treatment was started.
Invisalign clear aligners arrive approximately one and a half months after they are ordered.
Treatment began in February 2023 when the patient started wearing the aligners.

This photo was taken early in treatment while wearing the aligners.

23.10
More than 80% of the extraction spaces had been closed.
This was nine months after treatment began.

23.12
After wearing all 47 aligners in the first set over 11 months, this was the result.
With Invisalign, patients usually visit the dental clinic every two months. During these visits, the dentist checks whether any attachments have come off and whether the teeth are moving properly.
If the teeth do not move according to plan, some areas may develop where the aligners do not fit properly.
During treatment, this patient also once extended the aligner change interval from the original seven days to 10 days when some areas were beginning to show an imperfect fit.
Overall, however, treatment proceeded well without any significant variables.
This was the result after wearing all 47 aligners in the first set.

23.12
The extraction spaces had all closed, but

23.12
there was a slight area of incomplete molar occlusion.
To improve the overall quality of the treatment result, additional aligners were remade.
The patient wore the additional aligners for approximately five months starting in January 2023, and treatment was finally completed in July of this year.

24.07
The dental midlines match precisely,

24.07
and the areas of incomplete occlusion were corrected with the additional aligners, resulting in good occlusion, which many patients are concerned about.

22.12~24.07
The total treatment duration was one year and seven months, which is less than two years.

22.12~24.07
Because the patient initially had relatively thin lips, the treatment plan was designed so that the front teeth would not be retracted too far. The lip profile after orthodontic treatment is favorable.

22.12~24.07
The front teeth, which had been protruding and tilted upward, now have a clean, straight angle.

The parallelism of the tooth roots is also favorable.
With Invisalign extraction orthodontics, the molars are prone to tipping toward the extraction spaces.
This is important not only at the treatment-planning stage but also during treatment, when close monitoring is needed to control this issue properly.
Moderation is important in every treatment.
These days, many people avoid an appearance in which the front teeth are retracted too far.
In particular, caution is needed because this can make nasolabial folds appear deeper or create a peanut-shaped face.

There are no orthodontic cases that cannot be treated well with Invisalign.
Please choose a dental clinic that is experienced in achieving good orthodontic results with Invisalign.
